Description

Working under the direction of the Director of Services, the Mobile Outreach Clinic Program Coordinator, using a harm reduction approach works within an interdisciplinary team to provide a broad range of holistic services to support, promote and empower individuals who are experiencing or at risk of homeless, marginalized and individuals living in social isolation within the Community to address barriers to accessing services.


Overseen by the Director of Services, the Programs Coordinator in this position is responsible for assisting in the ongoing development and the day-to-day management of the Mobile Outreach Clinic.

The Program Coordinator will collaborate with the team to ensure that community members are aware of the Covid

  • 19 vaccination as well as provided with information resources, referrals, and transitions to services while continuing to build/ consolidate partnerships with a wide variety of stakeholders.
The Programs Coordinator requires the ability and skills to work with Community Members who may be oppositional and/or volatile, while at all times maintaining composure and behave in a professional and dignified manner


The Programs Coordinator will ensure that all activities are undertaken in a manner that is consistent with and complementary to the Vision Statement, the Principles and Values, and the policies and procedures of the Aboriginal Health & Wellness Centre.


DUTIES & RESPONSIBILITIES
Responsible for the overall management of the supports offered through the Mobile Outreach clinic.

Support staff working within the Community / Outdoors (Shelters, Encampments etc.).Emphasize Culturally sensitive person-centred care

Approach all situations in a safe, non-judgmental manner

Closely work in collaboration with the Immunizers/ FIT Team for the clinical services to support flow and vaccination operations

Responsible for the overall inventory management and distribution of social supports such as food, cultural care packages, transportation, fact sheets, and activity kits

Support both on-site and community outreach pop-up vaccine sites

Responsible for facilitating the Covid-19 vaccination administration for First Nation, Métis and Inuit individuals and community members

Work in collaboration with the Communications Coordinator to create and distribute information on vaccination to community organizations that can reach out to target population

Coordinate and conduct daily morning team huddles

Coordinate with the Immunization FIT Team on site to oversee operations emphasising culturally sensitive person-centered care

Ensures staffing schedules for all AHWC community roles are planned and managed according to schedule needs for each day

Ensures the flow of community member is safe and efficient throughout the day when access on-site pop - up vaccination days

Ensures all safety precautions are taken for cleaning both van and program areas

Works with AHWC Leadership in charge of logistics and/or specialized reporting functions and reports operational issues as advised.

Coordinates communication updates with staff

Trouble-shoots problems/concerns that arise, including complaints

Utilization of a "Harm Reduction Approach" to support community members

Monitor, locate, and identify, individuals that need support in accessing services

Promote health equity and education to support community member's basic health needs and Covid-19 uptake

Support clinicians on team to deliver COVID-19 community vaccination support in accordance with Regional Health and Provincial COVID-19 Vaccination protocols, AHWC's vaccination program standards, policy and procedural operations, and organizational values

Follow Manitoba and/or Shared Health Inc.'s staff instructions respecting security, use, storage and destruction of Personal Health Information Act (PHIA) including but not limited to ensuring all records of PHIA must not be removed from vaccination site premises

ADMINISTRATIVE DUTIES
Collaborate with administrative personnel regarding staffing, time sheets, and administration functions of vaccine site

Communicate supply needs and ensure inventory is ordered, stocked, and maintained

Oversee the pop-up distribution inventory management and supply chain etc.

Identifies areas of needed policy and procedures or revisions and bring them to attention of the AHWC Leadership
