Our Village of Coleman Care, located in Barrie Ontario has an exciting opportunity for a
RESIDENT SUPPORT COORDINATOR (known as a Social Worker or a Social Service Worker) to join their village team
Full-Time, 75 hrs Biweekly.

As our new Resident Support Coordinator, you will:

  • Have a degree in Social Work or related field such as Social Service Worker with a speciality in gerontology
  • Registered with the Ontario College of Social Workers & Social Service Workers
  • Coordinate and support the transition of residents moving into a health care setting
  • Support resident and family meetings, care conferences and therapeutic groups
  • Provide education, training, coaching and counselling for residents, families and team members regarding mental health and wellbeing
  • Support residents on the waitlist and ongoing assessment needs with residents and those waiting for services

It would be an asset if our new Resident Support Coordinator has:

  • Current Heart Save Level of CPR
  • Knowledge of relevant policies, procedures, legislation and regulations applicable to the longterm care sector, including the Fixing Long-Term Care Act, 2021 and Health & Safety Act of Ontario
  • Demonstrated experience working with seniors and/or persons with cognitive or physical impairments

As a new Resident Support Coordinator, you will:

You will work as key members of our Village and neighbourhood multidisciplinary teams to understand residents' needs, and advocate for residents to actively participate in decisions about their physical care.
