Case Manager - Vancouver, Canada - YWCA Metro Vancouver
Description
Under the general supervision of the Case Manager Coordinator, the Case Manager is responsible for working closely with job seekers to assess service suitability and develop and implement employment action plans.
Responsibilities:
- Work as part of a multifaceted and multiagency team to deliver a comprehensive employment program designed to meet the individual needs of clients
- Work within a performancebased contract
- Maintain an organized and uptodate caseload, including managing regular client followup
- Assess client needs and develop an individualized action plan to meet identified employmentrelated goals
- Use the Integrated Case Management system (ICM) to report client progress and milestones
- Administer Client Needs Assessments and Disability Related Employment Needs Assessments
- Identify the need for formal assessments such as Vocational Psychological Assessments, Functional Capacity Evaluations, Neuro-Psychological Vocational Assessments, and Audiological Reports
- Assist Clients in accessing appropriate interventions such as workshops, training, selfemployment, wage subsidy, counselling, job development and customized employment
- Understand and follow all policies and procedures related to WorkBC Case Management and stay current with updates and changes in policy
- Work Closely with the Quality Assurance and Office Manager to ensure accurate and consistent documentation
- Adhere to onsite Health and Safety Procedures
- Other duties as assigned
Qualifications:
- Relevant postsecondary degree or diploma or a combination of relevant education and experience
- Familiarity with the Integrated Case Management system (ICM) and WorkBC policies is a strong asset
- Case Management and/or employment counselling experience is an asset
- Excellent verbal and written communication skills
- Excellent computer skills, including Microsoft Office suite
- Ability to speak a second language is an asset
- Passion for helping jobseekers achieve their goals
- A valid criminal record check (including working with vulnerable populations) is required
- Successful applicants are required to be fully vaccinated against COVID19; exemption requests must be supported by valid legal documents
