POSITION OVERVIEW


As a Paramed Leader you are responsible to lead, supervise and engage unregulated care team members in the effective delivery of patient services.

You are skilled in troubleshooting complaints from patients, funder and other service providers.

These responsibilities shall be carried out in accordance with ParaMed's mission statement, policies, relevant legislation, government guidelines, Collective Agreements and local contract requirements.

ACCOUNTABILITIES
LEADERSHIP - 30%

  • Leads, supervises and monitors work performance of team members
  • Empowers and guides team members with regular coaching and mentoring
  • Drives engagement and retention initiatives that motivate team members to deliver quality care
  • Applies active listening techniques, questioning skills and empathy to uncover and address team members strengths and development needs
  • Hires, onboards, trains and manages performance of team members; includes documentation of performance reviews for all new hires
  • Proactively monitors, assesses, and addresses employee performance concerns in a timely manner through inhome visits, internal/external feedback, funder feedback
  • Receives and responds to team members workrelated concerns/issues in a timely manner.
  • Effectively raise concerns and questions to Care and Services Manager
  • Collaborates with Coordinator to approve team member's vacation
  • Hosts regular team member meetings, following a standardized agenda
  • Knowledge of CA agreements and applicable ESA standards
CUSTOMER SERVICE & RELATIONSHIPS - 20%

  • Effectively addresses and manages patient needs and complaints and executes action plans to successfully resolve complaints
  • Identify ways to improve customer service in collaboration with Care and Services Manager to determine standard procedures and develop solutions to reduce number of complaints
  • Investigates and responds to funder/patient complaints/concerns
  • Communicates regularly with funder about patient Plan of Care, changes in condition and risk issues.
  • Develops and strengthens a positive working relationship with patients, funder and other service providers.
  • Actively participates in internal and external committees and community functions (i.e. workshops, job/health fairs, patient conferences, provider meetings, participation on focus groups).
  • Responds to requests for private/another funder service
QUALITY CLINICAL PRACTICE - 25%

  • Assess, evaluate and validate team member's skills are met to provide the best quality care
  • Ensures establishment of an effective patientrelated Plan of Care which is consistent with funder service plan, relevant legislation and government guidelines
  • Conducts inhome visits to assess quality care, patient satisfaction and provide mentoring to team members
  • Works collaboratively with interdisciplinary teams, other internal and external health care teams to ensure that patients receive required care
  • Receives and reviews referrals/requests from funder to determine risks, training and education
  • Case manages private funded patients
  • Determines appropriateness of an unregulated care provider performing routine and advanced skills
  • Ensures team members are appropriately trained to perform advanced skills
  • Report to funder changes in patient status and care requirements
  • Attend case conferences as required
RISK MANAGEMENT - 10%

  • Responds to and follows through on all reporting and potential and actual risks regarding patient and team member safety
  • Ensures team members are informed in a timely manner of any potential or actual risk
OPERATIONS - 5%

  • Provides advice and support to Coordinators, as required, when scheduling for a nonroutine/complex patient
  • Awareness and understanding of our KPI's and participate in achieving targets (internal and external)
  • Assist both Operations and Human Resources to provide insight on staffing needs and gaps
  • Works collaboratively with Operations for team member's compliance on clocking in and out and chart completion
RECORDS MANAGEMENT - 5%

  • Ensures compliance with ParaMed's documentation standards for all patient records
  • Conducts regular quality review audits to ensure compliance with documentation standards
  • Responsible for ensuring completed charts are returned upon discharge
REPORTING AND DATA ANALYSIS - 5%

  • Collaborates with Care and Services Manager to review gaps, trends, and makes recommendations
  • Report weekly to CSM on personal metrics (i.e. completion rates of performance management reviews)

QUALIFICATIONS

  • Registration/licensing in good standing as a Registered Practical Nurse/Licensed Practical Nurse with provincial regulatory college
  • Ability to manage, negotiate and influence while having critical conversations with team members, patients, and operations by using strong verbal and written communication skills
