Social Worker/Social Services Worker - Extendicare Laurier Manor

RESPONSIBILITIES:

Participates in resident admission process including tours, admissions, transfers, discharges and occupancy tracking. Works closely with the LHIN on a daily basis
Acts as liaison with families and advocates for residents, encouraging them to exercise their rights
Participates in the interdisciplinary team process of resident assessment, care planning and evaluation to ensure services are planned according to individual needs
Consults with psychogeriatric outreach team when needed in addressing behavioural concerns with residents
Participates on committees related to resident care and operation of the facility as required
Liaises and consults with community partners, company resources and professionals in relation to resident care needs and facility activities
Provides therapeutic services through individual counselling in a variety of areas; adjustment to new environment, changes in health status, coping with feelings of anger, sadness, guilt, loss and grief
Addressing resident and family complaints and referring to appropriate departments as needed
Working closely with the business office regarding resident finances, POA issues and capacity assessments
Promoting a positive atmosphere and attitude and playing a leadership role
Other duties as assigned with in scope

QUALIFICATIONS:

Degree/Diploma in Social Work or Social Service Worker from an accredited University/College
Registration with Ontario College of Social Workers and Social Service Workers. (OCSWSSW)
Previous experience in longterm care and knowledge of MOHLTC regulations an asset
Excellent communication, clinical assessment, supportive therapy, crisis intervention and grief counselling skills
Experience in working with geriatric populations and dementia, depression, responsive behaviours and other psychogeriatric issues
Strong computer skills with an understanding of RAI-MDS and PointClickCare software an asset
