Coordinator, Planning and Systems Development to join our Primary Health Care team in

Surrey, BC.
Working closely with clinical leadership and management teams, internal and external stakeholders in this key role, you will:

  • Lead activities and projects that will facilitate system-wide process improvement,
  • Coordinate evidencebased planning and design of assigned initiatives;
  • Determine, develop and evaluate processes, guidelines and pathways for related initiatives;
  • Monitor and provide input into budget and resource allocation;
  • Develop and maintain appropriate documentation; prepare reports, graphs, tables, briefing notes, presentations and issue papers, and will make recommendations based on the data.

Supporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients/clients/residents and families:

The Coordinator, Planning and Systems Development, working closely with designated areas, clinical leadership and management teams, as well as internal and external stakeholders, is responsible for leading, planning, designing, implementing, maintaining and evaluating assigned initiatives that will facilitate system-wide process improvement.


Responsibilities:


  • Coordinates the evidencebased planning and design of the assigned initiatives.
  • Determines, develops and evaluates processes, guidelines and pathways for initiatives related to the assigned area(s) that ensure high quality patient care, in collaboration with clinical leadership, medical/physician leadership (as required) and other clinical areas as required.
  • Leads the implementation, monitoring and evaluation of initiative activities, ensuring adherence to systems, practices, policies and timelines.
  • Monitors and provides input into budget and resource allocation for assigned initiatives, ensuring their efficient utilization within the defined objectives, plans and budgets.
  • Leads teams to achieve deliverables and meets with key internal and external stakeholders to ensure effective communication at all critical stages of initiatives.
  • Develops and maintains appropriate documentation including action and decision logs. Prepares reports, graphs, tables, briefing notes, presentations, issue papers and makes recommendations based on the data.
  • Liaises with organizations, agencies and the communities to identify and document deliverables including issue discussion and resolution, in support of efficient and effective execution of the initiative.
  • Liaises with appropriate stakeholders to ensure that the coordination of information is maintained; prepares reports and planning documents.
  • Monitors progress and takes corrective action to ensure that initiative reporting requirements are met.
