We are looking for a
Care Team Coordinator to join our team. Our
Care Team Coordinator is at the hub of our service.

They are the reason why we can connect our healthcare workers and clients, and an important part of our equation of why our clients can live at the comfort of their homes safely and be given access to quality care.

This
Care Team Coordinator role is a Full-Time opportunity based out of our Guelph office.


What you will be doing:


  • Handinhand with our frontline workers, you manage and support the schedules of our PSWs and Nurses which is crucial to ensure our clients receive quality care.
  • You are client and customerfocused. Customer Service Skills are part of your DNA. You partner with your clients, their caregivers and families to understand their needs and connect them with healthcare workers that can provide them with the best care.

Skills needed for success:


  • Extensive experience in customer service and scheduling, preferably in a healthcare environment.
  • Comfortable using technology and MS office.
  • Attention to detail and efficient in data entry from previous administrative experience.

$33,150.00 per year
