PERSONAL SUPPORT WORKER - DAYS
The Director of Care is responsible for the overall nursing operations for the home.

This position reports directly to the Registered Nursing Staff.

The Registered Nursing Staff reports directly to the Assistant Director of Care who reports directly to the Director of Care.

This position has no staff reporting to them.

Nursing and Personal Care

  • Take morning report from the RN/RPN on the unit prior to starting their shift.
  • All staff working the full 8 hour shift must carry the pagers with them at all times.
  • Staff must be familiar with the residents that they are caring for. This includes reading the resident care plans to obtain specific information about the resident.
  • Assist residents with A.M. care which includes toileting, washing and dressing, mouth care including cleaning of dentures.
  • Empty commodes if left from night shift and tidy rooms.
  • Prepare residents for meals including portering residents to and from the dining room on each resident home area.
  • As many residents as possible should eat in the dining room. Tray service should be provided as the exception and there must be a documented reason for the tray service to occur.
  • Trays are distributed and residents who remain in their room for dinner are fed once all residents in the dining room have been served their meal.
  • The dietary aide prepares the plates for each individual resident and the PSW/HCA serves the residents their meals, course by course. Residents are assisted, supervised and fed when and if necessary.
  • Help feed residents requiring assistance both in the dining room as well as those residents who must remain in their rooms for meals.
  • The HCA / PSW's on each resident home area is assigned to the following dining room duties:
  • ensure that residents are comfortable and bibs are in place for those residents that require them;
- after the first course of the meal has been served, the HCA / PSW assists the residents by cutting up meat, buttering bread etc. as necessary (PSW / HCA's are not to enter the servery / kitchen area to get food);
- assist residents back to their rooms after meals.

  • After breakfast and lunch complete all food / fluid flow sheets for the residents you have assisted in order to ensure accurate information is obtained.
  • Toilet and change incontinent residents and those requiring assistance as required by the residents condition / preferences / needs and documented care plan.
  • Provide assistance to residents to prepare for and attend all activities and religious programs of their choice.
  • Serve morning and afternoon snacks and nutritional supplements. Wipe cart clean after distribution.
  • Bathe residents as per schedule and check skin, fingernails and toenails. Fingernails are to be cut by PSW staff. Report any concerns to the charge nurse immediately.
  • PSW staff is not permitted to cut toenails. Residents who require care of toenails should be reported to the Registered Nursing staff on the unit.
  • Change bed linen as required.
  • Maintain personal care carts in a clean and tidy manner. Replenish supplies as necessary for oncoming shifts.
  • Tidy resident closets and drawers as necessary.
  • Assist residents who require a rest period in the afternoon to get into bed and get comfortable.
  • Call bells must be attached to the bed within easy reach of the resident and must be answered in a timely manner.
  • Turn and reposition dependent residents at a minimum of every 2 hours.
  • Complete rounds of the unit prior to the end of your shift to ensure all residents are safe and comfortable.
  • Complete PSW flow sheets each shift for the residents that they have cared for only.
  • Complete routine safety rounds of the unit at least hourly to account for all residents.
  • Assist in providing information to the RN/RPN relevant to the MDS process.
  • Report immediately any concerns relating to resident behavioural issues.
  • Ensure that there is staff replacement prior to you leaving your shift. Report immediately to the RN/RPN if a staff replacement has not arrived.
  • Perform any other duty as required and/or assigned.
  • Note: The 0600hr day shift position must assist night shift from with completing personal care for those residents who require two staff assistance.
