An exciting Patient Care Manager (PCM) opportunity exists to grow your leadership skills & join a dynamic team within the Southern Alberta Transplant (ALTRA) portfolio.

The PCM is a key leadership role in assuring excellence in patient
- family centered & multidisciplinary care for complex renal & transplant patient populations across inpatient, outpatient & community care. The PCM leads & manages the following areas within the Foothills Medical Center (1) Unit 37- highly complex & acute med surg inpatient unit, supporting renal transplant recipients, living donors & complex nephrology patients (2) Peritoneal Dialysis (PD) Outreach team which supports FMC inpatient & Emergency care (3) ALTRA clinics across all organ groups - heart, kidney, lung, liver, pancreas (4) Living Donor Program. The PCM leads the ALTRA clinic teams in providing pre
- & post-transplant care for all recipients & living donors for Southern Alberta, in collaboration with Southern Alberta Organ & Tissue Donation Program & the Northern Alberta Transplant teams. The PCM reports to the Executive Director (ED) of Alberta Kidney Care South & Southern Alberta Transplant Program, working in a decision-making & accountability relationship with the ED, Unit Managers, & medical dyads. The successful PCM will possess an understanding of patient family-based care strategies & will strive to build inclusive environments that reflect the needs & supports required by teams & patients. The PCM will possess excellent time management skills, the ability to exercise sound judgement utilizing an evidence-based approach, strong interpersonal communication skills, quality improvement & change management skills, the ability to build partnerships & strategic plans with internal/external stakeholders within a complex health system while also overseeing transplant regulations based on accreditation standards.

The Patient Care Manager (PCM) position is responsible to promote & maintain a healthy work environment; demonstrate leadership skills with human resource functions including recruitment, coaching, clinical supervision, scheduling, performance appraisal management within multiple union environments.

The PCM will establish & maintain excellent working relationships with physicians, other health care providers & program areas in support of patient care & the vision, mission, values & business plan of Alberta Health Services.

The successful incumbent will be expected to demonstrate proficiency in the following areas:
Consistently role models AHS values. Ability to balance multiple, competing demands & conflicting priorities. Excellent analytical & decision-making skills, based on a framework of patient & family centred care, safety, & quality improvement. Effective facilitator of proven change management principles within teams, & ability to motivate others to achieve organizational goals.

Leadership skills, including demonstrated ability & comfort with decision making responsibilities, coaching, & the ability to inspire & build confidence in others.

Required Qualifications:

Baccalaureate degree in Nursing, or equivalent education considered. Minimum of 3 years in a management/clinical leadership role. Previous leadership experience including decision-making, coaching, mentorship, & teaching.


Additional Required Qualifications:

Previous experience in a healthcare organization of significant size & complexity. Previous Experience with stakeholder engagement, consultation & facilitation. Previous experience working with internal & external stakeholders. Previous leadership experience in an acute in-patient, outpatient &/or community care settings. Experience & proficiency with Microsoft office tools including PowerPoint & Excel.


Preferred Qualifications:

Master level preparation in Nursing or Administration. Experience in leading/supporting quality improvement projects & facilitating meetings. Demonstrated progressive leadership development including completion of leadership courses. Completion of Quality Improvement certifications (i.e., AHS AIW Yellow Belt, Green Belt, Black Belt or equivalent).
