    Oceanside Primary Care Network Project Lead

    Background:

    Divisions of Family Practice are an innovation in health care in BC, designed to support and advocate for family physicians, increase primary health care capacity and improve patient health outcomes. The Central Island Division (CIDFP) of Family Practice is a non-profit organization governed by a board whose members are primarily family physicians in the Parksville, Qualicum Beach, and Port Alberni areas (geographic area).

    A Primary Care Network (PCN) is a clinical network of local primary care service providers located in a geographical area, with patient medical homes (PMHs) as the foundation. A PCN is enabled by a partnership between Divisions of Family Practice, Indigenous organizations, community stakeholders, and the health authority.
    Job Title: Oceanside Project Lead, Primary Care Network
    Reports to: Primary Care Network Director
    Status: Contractor
    Hours: Up to 37.5 hours hrs per week, with some flexibility required to support early morning and evening meetings.
    Term: 1 year, with strong possibility of renewal
    Pay rate: $100,000-$110,000/annum
    Closing Date: May 31, 2024

    Contract Summary
    The Primary Care Network Project Lead's focus is to work with the team to ensure that the key deliverables set out in the Primary Care Network Service Plan are completed. In carrying out these duties, the Project Lead will develop reports, ensure the timely and effective implementation of decisions, work on specific deliverables detailed in the service plan, and analyze and prepare information to achieve results. The Project Lead will provide and engage in a wide range of activities, including but not limited to:

    1. Lead/support project planning and development activities, including working with capital planning teams to manage new clinical builds through leasehold improvements for new and incoming clinical services planned through the PCN
    2. Support the integration of allied health professionals, Indigenous wellness providers, RNs and clinical pharmacists into patient medical homes and primary care teams
    3. Complement other practice supports available, working within patient medical homes to provide coaching and advisory services for point-of-care teams on how to manage change and develop team-based care
    4. Facilitate interdisciplinary, multi-partner meetings to co-design culturally safe, team-based care service delivery with physicians, nurse practitioners, Indigenous partners, health authority partners and other community stakeholders.
    5. Facilitate strategies that will enable participants and project teams to apply engagement strategies, and change management theory to achieve project aims and/or targets
    6. Monitor project progress and finances as required; recommend modifications to project plans and/or budgets as needed and lead any refinement processes and report on same to PCN Director
    7. Coordinate the duties and responsibilities of the program(s) team(s) and working group(s) to fulfill the project deliverables, and meet regularly to provide support, monitor progress and participate in project activities in line with best practice project management
    8. Prepare monthly progress reports to the PCN Director and PCN Steering Committee on all project activities and assist to develop internal reporting forms and processes.
    9. Meet regularly with the PCN Director and PCN Physician Lead to confirm direction, to provide project updates, and to escalate issues or concerns that arise from the delivery of the project that may be of interest or importance to project teams and/or the Board of Directors.
    10. Collaborate with other contractors and staff of the CIDFP, Island Health, FNHA and community organizations.

    Specific Accountabilities
    The Project Lead will work on contract from their home office, attend regular meetings, perform job-related duties in the geographic area, as well as potentially attend out of region meetings. Attendance at early morning or evening meetings will sometimes be required. The incumbent will hold a valid BC driver's license, have access to a reliable vehicle, and will be supplied with a laptop computer and cell phone to use for business purposes.

    Qualifications
    • A level of education, training, and experience equivalent to an undergraduate degree in a related discipline. And/or;
    • PMP or Change Management Certification would be an asset
    • At least ten years of recent relevant experience successfully leading change in a complex environment, organizational learning development, and community engagement. Experience with BC primary healthcare system is required.
    Desired Skills and Experience

    1. Preference will be given to candidates with clinical experience and/or experience with clinical operations
    2. Preference will be given to candidates with experience in capital planning of primary care facilities
    3. Ability to develop trusted relationships with multiple partner clinics and service providers to enable best practice change management methodologies
    4. Experience with charting and electronic medical records, specifically, MedAccess EMR
    5. Experience developing clinical workflows
    6. Practical experience in managing multiple and complex projects

    17. Ability to work with key stakeholders to identify their expectations and shared objectives
    18. Experience in team dynamics, including ability to mentor participants in successful team management
    19. Inspires others to find new ways of working together to provide excellent primary care
    20. Maintains a thorough understanding of local, regional and provincial primary care
