Position Description:

***: The Dual Diagnosis Transitional Rehabilitation Housing Program and Outreach Service provides services for people with developmental disabilities and concurrent mental health disorders who are in the Forensic system or at risk of entering the forensic system. Working in collaboration with the Ministry of Community and Social Services and a designated housing provider, the Liaison Worker is instrumental in facilitating communication between the housing facility and the hospital; and, coordinating the processes required to provide education and consultation to assist Developmental Sector agencies to work with and risk manage persons at risk of involvement in the justice system. The Liaison Worker will also be involved in the evaluation of this new initative as well as participating in research generated by this new model, as appropriate.


QUALIFICATIONS:


  • Bachelor's Degree in Nursing, Social Work or related field required; Masters Degree preferred
  • Regulated health professional (Social Worker, Registered Nurse or related health professional)
  • Professional membership preferred
  • Certification with CPMHN preferred
  • Maintains current working knowledge of relevant legislation, including: Mental Health Act, Health Care Consent Act, Substitute Decisions Act, Public Hospitals Act, Long Term Care Act, Child and Family Services Act, PHIPA, and Excellent Care for All Act
  • Knowledge of the Criminal Code of Canada
  • Knowledge of the Occupational Health and Safety Act and Regulations, professional standards and guidelines, SJHH corporate and departmental policies and procedures, including successful completion of WHIMIS training
  • Knowledge of documentation standards, privacy legislation, and health record management processes
  • Demonstrated knowledge and experience working with mental health client populations served
  • Demonstrated education and facilitation skills
  • Excellent verbal and written communication skills
  • Demonstrated ability to work as an effective member of an interprofessional team with exceptional interpersonal, advocacy, negotiation and conflict resolution skills
  • Exceptional critical thinking and creative problem solving skills
  • Demonstrated knowledge of the assessment and management of comorbid medical disorders e.g. diabetes, metabolic monitoring etc.
  • Demonstrated knowledge of developmental delays and interventions to reduce negative behaviours and associated risks
  • Demonstrated knowledge of MSE (mental status examination)
  • Demonstrated knowledge and skill related to crisis intervention and conflict resolution
  • Demonstrated leadership, problem solving, critical thinking and strong team building skills
  • Demonstrated knowledge of interpersonal and deescalation skills
  • Demonstrated knowledge of psychopharmacology, including current psychotropic medications and side effects
  • Demonstrated ability to work under pressure, prioritize workload, and meet deadlines
  • Proficient computer skills for report writing, assessments word processing, workload measurement, and access to information
  • Demonstrated ability to work independently and show initiative along with anticipatory problem solving
  • Demonstrated understanding of Forensic Patients supervised under the Ontario Review Board, reporting requirements and risk assessment
