4Korners is a registered Canadian charitable community-based organization founded in 2005.

We connect with English-speaking individuals of the Laurentians with programs and services to meet their identified needs in a secure and inclusive environment.

We work collectively to serve and enrich the lives of communities in the Laurentians region.

4Korners offers a wide range of programs that focus on healthy lifestyles, mental health, foster creativity, and provide support for youth, families, individuals, seniors, and caregivers.

During the past years, 4Korners has expanded with new programs throughout the Laurentians.

Along with our Head Office in Deux-Montagnes, we run an Activity Centre in Deux-Montagnes and have four satellite offices in the region with multiple services in many small towns.


With a unique and ambitious mandate covering a large territory, we are looking for a professional and passionate
Seniors and Caregivers Coordinator to join our team.

The Seniors and Caregivers coordinator will be responsible for leading a
Community of Practice for caregivers and seniors by building their capacities and resilience.

While adhering to the overall mission and values of the organization, the position will work toward social engagement and thriving communities.

They will also work closely with community to identify and relay the needs of seniors and caregivers to different stakeholders, while representing 4Korners at various tables or committees in the Laurentians.


As a hybrid position, work will be required at the office of assignment, in community spaces, tables and committees in Deux-Montagnes and Thérèse-de-Blainville, and virtual.


OBJECTIVE


To lead a community of practice for caregivers and seniors in the Laurentians, where common needs and concerns are identified, knowledge and best practices are shared while building capacities and resilience in these communities.


RESPONSIBILITIES

Caregiver and other Community Support Activities - 40% of hours

  • Coordinate workshops oriented to caregivers to share knowledge, best practices, and build capacity.
  • Coordinate and lead monthly caregiver support groups.
  • Offer individual or group support and intervention to caregivers as needed, or by connecting them with relevant resources.
  • Supervise, mentor, evaluate and oversee interns working in social services placed in Deux-Montagnes, in collaboration with the Seniors and Caregivers program manager.

Seniors Wellness Activities: 30% hours

  • Coordinate and facilitate virtual and inperson activities for seniors in Deux-Montagnes (and possibly in ThérèsedeBlainville) as described in the funder workplan.
  • Support seniors as needed, or by connecting them with relevant resources.
  • Coordinate, supervise and support volunteers.

Networking & Partnership Initiative Outreach: 15% hours

  • Represent the Englishspeaking community and 4Korners on tables and committees (_i.e., Table de troisième âge, Comité proche aidants, Comité abus_).
  • Represent 4Korners with a kiosk and at events prioritized by 4Korners.
  • Create and equip partners with knowledge, information and support while working collaboratively with them in making improvements.
  • Respond and direct community to resources, and refer them to other team members or partners, as appropriate.

General Administration: 15% hours

  • Participate in mandatory trainings and team meetings.
  • Ensure that all required reporting is tracked and submitted by deadlines.
  • Support the Executive Director and the Seniors & Caregivers Program Manager.

QUALIFICATIONS

  • A Bachelorslevel degree in a health profession, social services, nursing,
    and/or a healthrelated field such as Gerontology.

OR
Relevant years of experience in working with seniors, caregivers, and supporting services in the community and mental health systems.

  • Experience working with older adults and their families, from a community proximity model.
  • Experience developing, running, and facilitating workshops and presentations.

ESSENTIAL SKILLS (mandatory)

  • Ability to communicate fluently in English. Working proficiency in French.
  • Excellent assessment, communication, case management, organizational and problemsolving skills.
  • Ability to build rapport and engage with seniors, families and caregivers.
  • Demonstrated ability to coordinate volunteers / interns.
  • Excellent skills in risk, crisis, conflict management and decisionmaking skills.
  • Active listening, respect, and discretion.
  • Demonstrated ability to work in a multicultural, diverse, multidisciplinary team environment.
  • Must have a car and be willing to drive.
  • First Aid and CPR training.
