SERDC Health is accredited by the Canadian Accreditation Council to provide exceptional 2nd level services to eight or more First Nations communities in Manitoba.

We are community-driven and are looking for motivated individuals who are willing to learn and make a difference in First Nations health.

We are seeking to fill

two (2) full-time positions.


Full-Time Position


Jordan's Principle - Child First Initiative is intended to ensure First Nations children do not experience denials, delays or disruptions of public services ordinarily available to other children due to jurisdictional disputes.

We are seeking fill two (2) positions.


Responsibilities:


  • Advocate and be a liaison for those in need of specific services and resources.
  • Provides one to one support when requested.
  • Consult with other Mental Health Professionals, Nurse(s), Physician(s), Psychologist/Psychiatrist for integrated treatment plan(s) as well as culturally appropriate holistic plan(s) of care.
  • Develop and oversee client care plans and ensure aftercare plans are followedup.
  • Assist in making referrals to other agencies.
  • Collaborate and network with other community resources, social service agencies and health care providers involved with clients to provide information and obtain feedback on clients' overall progress.
  • Review and approve engagement materials including PowerPoint presentations and written material specific to Jordan's Principle and related topic(s) of interest.
  • Provide specialized case management and community outreach with Jordan's Principle staff and partners ensuring all children identified with health needs receive services when off reserve.
  • Prepare and provide reports to meet deadlines as requested.
  • Record information gathered for reporting & quality improvement purposes.
  • Establish and nurture partnerships, to ensure a comprehensive referral network for community resources and clients.

Qualifications:


  • Degree or Diploma in Social Services, Health, Social Work, Social Science, Human Services or equivalent.
  • Minimum 5 years' experience working with First Nation families and communities.
  • Knowledge of community resources on and off reserve available to clients and their families.
  • Strong conflict resolution skills, an understanding, sensitivity, and cultural safety and awareness of the social and cultural history of Indigenous peoples.
  • Valid Emergency First Aid/CPR certification.
  • Strong advocacy skills to advocate on behalf of families/clients.
  • Ability to use tact and good judgment in dealing with sensitive and complex issues.
  • Ability to work independently and as a member of a multi service team.
  • Excellent verbal and written communication skills are essential.
  • Expertise with case management and the development of integrated case plans.
  • Knowledge and experience working with children with special needs.
  • Must have a valid class 5 driver's license and reliable vehicle.
  • Ability to travel.
  • Individuals must be double vaccinated and provide proof of vaccination

Desired:


  • Ability to communicate/speak in a First Nations language(s) (i.e., Ojibway/Cree/Oji-Cree, etc.) and/or previous exposure to First Nations communities and cultures would be a definite asset.

Conditions of Employment:

  • Must produce a satisfactory Adult Abuse Registry Check
