Scope and Summary
of Primary ResponsibilitiesThe position requires a highly competent and well-organized health care professional who values safety, security, and Client wellness above all else. This position is responsible for completing a broad range of responsibilities and duties that will enhance the provision of Vision Loss Rehabilitation Services. The primary responsibilities include working in collaboration with our ACVREP Certified Rehabilitation Specialists and Occupational Therapists to carry out established rehabilitation activities and/or programs.
II. Essential Functions
This position entails, but is not limited to the following general responsibilities:
•Under the direction of the Occupational Therapist/Rehabilitation Specialists, receives referrals and assists with the implementation of rehabilitation plans/program.
• Actively contributes to the development of BC-wide student Occupational Therapist resource-guide and education opportunities.
•Supports the delivery and contributes to the coordination of information sessions for external stakeholders.
•As part of a multidisciplinary team and in consultation with the therapists/specialist, supports planning, implementation, modification, and evaluation of the rehabilitative treatment plans.
•Demonstrates and reinforces adaptive skills for independent living, in areas of personal management, household management, communication, basic indoor orientation & mobility skills utilizing vision enhancement tools, basic keyboarding and technology training.
•Assists the therapists/specialists with the maintenance and inventory of adaptive devices and loaner equipment programs.
•Provides functional direction to the client and other collaterals to maximize the client's rehabilitation process.
•Maintains all case data and documentation consistent with VLRC Systems and Procedures.
•Collaborates with rehabilitation team in service delivery, coordinates travel schedules and facilitates program development.
•Other duties as required

•Provides appropriate support and consultation to, clients and their families.
•Contributes as part of multidisciplinary team.
•Supports the development of acute-care and rehabilitation setting relationships.

•Utilizes established business rules, service standards and practices, and ensures policy and procedures are followed in a timely fashion.

•Participates in service evaluation and program planning processes, as needed, to maintain and improve service outcomes.

•Works under the general direction of national/divisional guidelines and standards but has latitude in making decisions on routine tasks.
•Consults with certified specialists or supervisor on issues that are more complex or may require upper-level approval.
•Exercises judgment and tact when working with Clients and knows when to elevate issues to the next level such as referring unusual client needs to appropriate person for service planning.
•Committed to maintaining privacy and confidentiality.
Leadership
•Takes initiative to keep abreast of new and changing community resources and VLRC policies & procedures.
•Facilitates and enhances team efforts on a professional level and shares specialized knowledge, resources, experience, concepts, and skills.

III. Qualifications
Knowledge and Skill Requirements
•You have 3 to 5 years client service experience in the health and/or social service sectors.
•You have experience working directly with people living with vision loss.
•You have knowledge of available community services/supports and clinical, social, and residential care options.
•You have an understanding of roles and linkages across primary care, community care and specialized geriatric services.
•You have knowledge of clinical expertise related to client centered, interviewing, assessments, and scheduling practices.
•You have experience working in settings requiring inter-professional collaboration.
•Excellent communication (verbal and written).
•You have exceptional interpersonal skills, including shared decision-making and facilitation.
•You can prioritize workload and manage competing tasks.
•You have the ability to take initiative and be resourceful.
•You are proficient in technology (e.g.: Microsoft office and case management and care coordination systems).
•You have demonstrated ability to work independently and within a team.
•You have expertise and experience in cultural sensitivity and diversity.
Experience and Education
•Post-secondary education and/or prior experience in a Rehabilitation Assistant program, Occupational or Physical Therapy Assistant program, Social Work or related field preferred.
•Previous working/volunteering experience working with individuals who are blind or partially sighted or a combined loss of hearing and sight.
•Clean Criminal Record required; Criminal Reference Check required for vulnerable populations.
