    Description

    As a Care Coordinator in the CBI Home Health Ottawa Office, your focus will be on ensuring a positive client and family experience by effectively scheduling direct care providers in the community to meet the requirements of client care plans. You will be part of a team of Care Coordinators helping others with compassion in care to facilitate continuity of care while balancing the pressure to deliver on scheduling requirements of both clients and care providers. Frequently problem solving, you will be the central point of contact for clients and families, care providers and referral sources.

    The salary for this entry-level position is $38,000 annually. You will be eligible for 3 weeks of paid vacation time and qualify for the group benefits program after 3 months.

    As a Care Coordinator, you will:

    • Use scheduling software to ensure continuity of care allowing care providers appropriate time for travel between client visits, in appropriate geographic areas, and within client and provider availability
    • Adhere to Employment Standards legislation, collective agreements and/or policies and procedures while carrying out care provider scheduling
    • Provide care providers with accurate schedules on a weekly basis, maintaining communication to ensure schedules are carried out
    • Effectively problem solve and communicate with clients, families and care providers when scheduling challenges arise
    • Ensure inquiries from clients, families and care providers are responded to and resolved in a timely manner
    • Utilize strong customer service skills to maintain positive relationships with care providers, clients and funders while considering individual needs in decision making
    • Maintain accurate information within databases such as scheduling and availability changes, care plans and client records
    • Ensure accurate and reliable reporting of data for analysis and monitoring trends and action plans
    • Liaise with Care Supervisors to share client and care provider feedback, escalating appropriately on concerns or risks for review and follow up.

    What you will need:

    • Hold a high school diploma or equivalency, Business or Medical Office Administration or similar education preferred
    • Strong customer service experience, 1 year call centre/ team service delivery experience preferred
    • Proficient computer skills in Microsoft Outlook, Excel & Word
    • Scheduling and/or coordination experience using software such as Alayacare preferred
    • Demonstrated excellent telephone etiquette
    • Ability to remain calm in stressful situations, empathize with the complexity and difficulty of each client and field staff situation
    • Detail oriented, adaptable and customer-focused team player
    • Combine strong interpersonal skills with analytical problem solving abilities
    • Self-directed, goal-oriented and able to manage time and priorities
    • Understanding of employment legislation, medical knowledge and/or background considered an asset
    • Bilingual in English and French, verbal and written language skills considered an asset
