Take your career to the next level in your exciting new role as Project Coordinator Reporting to the Executive Director - Seniors Health, you will develop and implement the Client Directed Care Program in different areas across the North Zone.

Client Directed Care is a service delivery option for acute care patients awaiting assessment for designated living options.

Funding is provided to clients to enable their return home, with the support of an AHS Home Care Case Manager and care services from an independent home care agency.

You will collaborate with Area Directors and Managers and Home Care Case Managers to identify opportunities and evaluate service delivery.

You will liaise with Alberta Blue Cross and the contracted agencies and participate in working groups related to Seniors Health, client directed care and other home care initiatives.

You will develop and maintain a tracking system, provide regular reports to stakeholders, and ensure the correct escalation of patient concerns.

Strong communication, interpersonal and organization skills are essential for this role.

Description:


Client Directed Care is a program that allows patients that do not require acute care services and cannot safely return home without additional supports, to do so through the use of agency services.

This Project Coordinator role will provide North Zone oversight of the Client Directed Care Program, including the facilitation and consensus building of North Zone review panel, consultation with Case Managers, and liaisons with Provincial Seniors Health Teams and Home Care Operations.

Liaise directly with Alberta Blue Cross and the contracted operators.

Develop and maintain strong working relationships with key contacts in AHS, with North Zone Operations, AHS Seniors Health Teams and colleagues in other AHS Zones.

Lead and participate on working groups and committees. Contribute to program development that supports the service delivery model, guiding principles and vision for Seniors Health. Be adaptable to changing workload, assignment and priorities.

Required Qualifications:


A minimum of an undergraduate degree in a health related discipline or other related area; equivalencies of education and experience will be considered; 3-5 years related experience in the field; 2 years' experience in Seniors Health (home care, supportive living and/or long term care); Excellent computer skills including advanced knowledge of Microsoft Word, Power Point, Excel, and Teams; Ability to travel throughout the zone (license required).

Training or experience in project management, program development, and/or change management.
