Position Overview:

ReLinC Services provides case management support to individuals with a history of serious and persistent mental illness. The successful incumbent will work in partnership with clients, families and the clinical team. They will meet with clients in their homes, in the community or in the office.

They will set individualized care plans with the client to improve functioning with activities of daily living; provide skills teaching; facilitate connections to community resources and aid with navigating medical and social assistance systems.

Support is dependent on the clients' specific needs and goals.


Key Qualifications:


  • Bachelors Degree or Community College degree in a health related discipline
  • Current registration with a recognized college
  • Minimum 35 years experience working with adult clients living with severe mental illness
  • Knowledge of psychosocial rehabilitation and recovery philosophy
  • Working knowledge of community resources
  • Excellent communication, organization, presentation and problem solving skill
  • Valid driver's license, relevant insurance and use of own vehicle
  • Demonstrated ability to work independently and as part of a team
  • Treatment planning and intervention experience required
  • Group facilitation skills
  • Experience working with clients with substance misuse issues is an asset
  • Ability to speak a second language is an asset
  • Proficient computer skills
  • Reliable, flexible and positive team player
  • Positive attendance record essential
