The Garry Hurvitz Centre for Community Mental Health at SickKids (GH-CCMH) is a child and youth mental health centre integrated with The Hospital for Sick Children (SickKids).

GH-CCMH is also a core service provider within the Toronto region of Child and Youth Community Mental Health Providers.

As such, the Centre has an ambitious agenda to provide the highest quality evidence-informed care offering a range of services from prevention and early intervention through counselling and therapy, assessment and intensive supports.

Services are provided in both in-person and virtual formats.

We are currently seeking applicants for a temporary full-time 1.0 FTE Case Manager.


This position is initially dedicated to the transition of children, youth and their families from the SickKids Emergency Department into GH-CCMH community based mental health services and supports.

The position will work collaboratively with partners at SickKids and at Strides Toronto to support the clinical transition pathway. The position may evolve to include other transitional pathways from acute to community care.

This position will provide service planning, navigation and brief supportive counselling to children and youth experiencing mental health concerns, and their families, to ensure they move in through and out of publicly funded mental health services in a timely and seamless manner.


Key Responsibilities

  • Provide developmentally informed transitional support for children, youth and families referred from SickKids to GH-CCMH.
  • Provide family education, counselling and support through brief interventions.
  • Facilitate multiple care aspects (e.g. referral review and consultation, intake management, case coordination, information sharing, implementation of practical clinical strategies, system/service navigation and support for informed decision making).
  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, patient and family centered and meaningful.
  • Adhere to established service level agreements regarding referral acknowledgement and intake.
  • Develop effective working relationships with relevant SickKids,GH-CCMH, and Strides colleagues and collaborate/communicate throughout the entire case management process.
  • Practice in a manner that is consistent with the philosophy of continuous quality improvement and evidencebased practice.
  • Prepare client documents and submit reporting information according to established procedures and deadlines.
  • Maintain uptodate knowledge of therapeutic services and best practices within the industry.

Qualifications

  • Master of Social Work or equivalent in a mental health discipline and a minimum of three years' related clinical experience or equivalent combination of education and experience
  • Membership in the appropriate professional college
  • Good verbal and written communication skills to provide therapeutic services to clients and families
  • Good problemsolving skills to analyze complex client issues and recommend solutions
  • Demonstrated skills in therapeutic engagement
  • First Aid/CPR is an asset
  • Good knowledge of MS Office Suites (Outlook, Word, Excel, PowerPoint)
  • Ability to incorporate values of a culturally competent practice of diversity and inclusiveness in all aspects of work
  • Ability to work flexible hours, including evenings and weekends

  • Concentration and creativity required daily to prepare documents, listen, attend meetings, mediate conflicts, develop transition plans and brief treatment plans, etc.
  • Sits at a desk and in sessions with clients in order to perform the duties of the job, as well as move objects or lift books for filing
