Job Summary:


In accordance with the Mission, Vision and Values, and strategic directions of Provincial Health Services Authority patient safety is a priority and a responsibility shared by everyone at PHSA, and as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.


Under the general supervision of the Shift Supervisor, the Rehabilitation Worker, Hawthorne Program, implements established programs and develops, implements and evaluates new, comprehensive Hawthorne specific group rehabilitative programs to meet the needs of Hawthorne patients using psychosocial principles and practices that facilitate the patient in a successful reintegration into the community.

Operates the Centre's passenger transportation vehicle to drive residents to designated locations.


Duties/Accountabilities:


  • Carries out components of an established psychosocial rehabilitative program to address the assessed needs of patients by implementing, actively participating in and evaluating open and closed therapeutic programs using psychiatric rehabilitation intervention strategies, and a wide variety of group-based rehabilitation programs such as ADL programs which include activities such as cooking, shopping, laundry and maintenance of living environment for patients; determining the destination and taking patients out for small Staff Supported Community Outings; and promoting, organizing and participating in patient leisure activities.
  • Works collaboratively with the multi-disciplinary team to develop individualized rehabilitative treatment plans for patients by identifying patient strengths, limitations and functional level for program involvement. Works with team members in the regular evaluation of assigned programs with regard to patients' assessed needs and progress, and modifies or changes programs as indicated, including recommending suitability for programs and determining when it is safe and appropriate to integrate community rehabilitation resources and opportunities into patient treatment plans.
  • As a member of the Hawthorne interdisciplinary treatment team completes rehabilitation assessments within designated scope of practice; maintains regular reporting relationships with nursing staff and clinical team members to promote and enhance patient care by observing, evaluating, and documenting patient behaviour and progress (attending treatment planning conferences as needed). Continuous observation, evaluation reporting and documenting changes in patient behaviour, performance and progress, to the nurse in charge in a timely manner Works with team members to adjust the treatment plan to reflect the patient's progress by evaluating on a frequent basis patient progress in relation to patient treatment plan to ensure priority needs are met. Assists in the preparation of individual discharge reports to meet organization documentation standards by summarizing information focusing on patients' community living skills and independence levels and recommending appropriate community supports where available. Maintains outcome and workload measurement statistics.
  • Works collaboratively with other departments and Treatment Team members to analyze the needs of Hawthorne patients as a basis for new program development, standards and evaluation strategies by reviewing relevant literature and recommending interventions that will support the achievement of desired treatment outcomes. Develops specific program curricula to address the functional level of the target population including researching, obtaining, and creating appropriate educational, testing, and other program materials to meet established program goals and objectives and evaluation processes.
  • Maintains work area and equipment by ensuring equipment and supplies, such as tools, materials and chemicals used in the program are stored appropriately and accounted for, arranging repairs as required, and assuming the responsibility for the purchase of supplies to support programs as required.
  • Performs regular security functions to maintain the safety and security of patients, staff, families and the general public at all times and takes immediate action to mitigate risks, as well as reporting all real and potential risk issues. Ensures safety and security of the staff, patients and general public during all planned activities such as Staff Supported Community Outings and supports the orderly conduct of patients using intervention/resolution strategies including NVCI, diversion, and supportive counseling as required and according to established procedures.
  • Drives residents to designated locations as scheduled, by operating the Centre's passenger van/bus with a seating capacity of 11 to 25 passengers, including the driver.
