What We Will Ask of You: As a key member of our Passport team, our Passport Coordinator plays an important role. On most days you'll be facilitating and monitoring funding agreements for approved individuals within the Passport Program. You'll be communicating with individuals awaiting services or funds with PassportONE on an ongoing basis while providing information and communication throughout the process. Supporting a caseload of approximately 500 individuals, you will be working closely with the Lead Passport Coordinator to ensure that best practices are followed and quality services are delivered. The Passport Coordinator additionally is involved in fostering community development among agencies and families by providing consultation, advice and orientation to the Passport Program.


Primary Responsibilities:


  • Supports the prioritization process for individuals who are waiting for Passport funding.
  • Manages a caseload of Passport clients as assigned by the Program Manager.
  • Communicates with clients and their families if applicable to provide support in using Passport funds to meet client goals.
  • Monitors client budgets and ensures that clients use funds within the Passport guidelines.
  • Ensures that client information in the database is current and accurate.

Qualifications:


  • Postsecondary degree in Social Work or a diploma as a Social Service Worker or equivalent.
  • 3+ years of social work experience.
  • Demonstrated experience of using best practice in supporting service users.
  • Developmental Services experience is considered an asset.
  • Strong attention to detail, well organized with an excellent ability to prioritize, multitask and take initiative.
  • Ability to maintain positive and professional relationships with parents and other stakeholders.
  • Computer proficiency and excellent communication skills.
  • Compassionate and responsible supporter of people, their families, and community partners.
  • Commitment to maximize resources and improve people's quality of life.
  • A valid driver's license and insurance, access to a reliable vehicle and ability to drive in various weather conditions.
