    OPPORTUNITY
    We are hiring a Full-Time Direct Care Worker to join our team. This position is an opportunity to put your knowledge and technical skills to work.


    THE ROLE

    Required to be a role model and support members to learn strategies that will keep the community and the members safe, keeping in line with treatment goals and strategies.

    Experience with data collection, Behavioral Support Plans, Risk Reduction Plans, Goal Setting and Programming in the residence and community.

    Implement Environmental modifications and Safety Measures to ensure the safety of members and staff.

    Support members with complex behavioural needs; some members have involvement with Center for Addiction and Mental Health (CAMH) and the Ontario Review Board.

    Develops, implements, and evaluates Personal Support Plans with the members' involvement to enhance their overall life skills and quality of life.

    Identifies members' need for specialized services; through consultations and referrals, arranges appointments, transportation (family visits, day programs etc.), and supportive accompaniment.

    Completes follow up documentation as necessary for specialized services

    Supports members in identified areas of work skills, recreational skills, social skills, and life skills, and when necessary, assists members with personal hygiene matters.

    Administers and records prescribed medication as per instructions from a medical practitioner and follows the agency's Medication Policies and Procedures.

    Communicates any medical needs of the member to the appropriate personnel, applies first aid as required and completes written follow-up documentation.

    Provides feedback regarding member's general health and safety issues to the Program Supervisor and completes documentation as necessary.
    Orders, administers, and records prescribed medications as per instruction from a medical practitioner.

    Ensures the residents' health, safety, welfare, and independence by completing and involving the member with general house cleaning, grocery & supply shopping, preparing meals and doing laundry as necessary.

    Assists in organizing leisure activities and ensuring community integration for all members in accordance with their needs and interests.

    Performs other related duties such as balancing petty cash, vehicle safety and maintenance checks, completion of fire drills, facilitating observation shifts and assisting with orientation of new members, staff, students, and volunteers.

    Participation in meetings and trainings as required.


    SHIFT:
    12-hour

    Rotating Shifts:

    Week 1:
    Saturday, Sunday, Wednesday and Thursday (9am - 9pm)


    Week 2:
    Monday, Tuesday, Friday (9am - 9pm)


    WHAT YOU BRING
    Developmental Service Worker (D.S.W), Social Service Worker (S.S.W), Child and Youth Worker (C.Y.W), Behavioral Science Technology or equivalent degree or diploma from an accredited college or university
    Attainment of a pharmacology certificate, safe management certificate and First Aid & CPR is preferred.
    Valid G or G2 Driver's License required.
