The Garry Hurvitz Centre for Community Mental Health at SickKids (GH-CCMH) is a child and youth mental health centre integrated with The Hospital for Sick Children (SickKids).

GH-CCMH is also a core service provider within the Toronto region of Child and Youth Community Mental Health Providers.

As such, the Centre has an ambitious agenda to provide the highest quality evidence-informed care offering a range of services from prevention and early intervention through counselling and therapy, assessment and intensive supports.

This position is initially dedicated to the transition of children, youth and their families from inpatient units at SickKids into GH-CCMH community based mental health services and supports. The position will work collaboratively with partners at SickKids to support the clinical transition pathway. The position may evolve to include other transitional pathways from acute to community care. This position will provide service planning, navigation and brief supportive counselling to children and youth experiencing mental health concerns, and their families, to ensure they move in, through and out of publicly funded mental health services in a timely and seamless manner.

KEY RESPONSIBILITIES

Clinical Services

As directed by, and in collaboration with Child and Family Therapist(s), provides clinical interventions and services for assigned children, youth and their families maintaining caseloads and supporting clients.
Participates in the implementation of individual treatment plans for clients assessing behaviours and responding to clients' needs.
Acts as a role model displaying ageappropriate behavioural management and intervention skills for clients.
Provides outreach and transitional support to clients, as required.
Coordinates cases with family/caregiver/relevant service providers/service planning team or creates materials for students relating to strategies/interventions.
Supports clients by facilitating social skills/therapeutic activities and organizing/attending events.
Prepares documents/reports related to treatment plans and related activities, and consults with others, as needed.
Maintains uptodate knowledge of best practices related to clinical services.
Prepares statistical data, as required.
Makes decisions related to daytoday treatment issues and ongoing intervention techniques.

Other

Complies with the organization's vision, policies and procedures.
Performs other tasks as assigned by management.

Qualifications

Postsecondary Diploma in Child and Youth Care and a minimum of one year of related experience or equivalent combination of education and experience; experience working in community is strongly preferred.
Member with the Ontario Child and Youth Care Association or equivalent association/accrediting organization.
Good verbal and written communication skills including communication skills with clients and families.
Good problemsolving skills to analyze client issues, recommend solutions and manage crisis situations
First Aid/CPR, and TCI.
Good knowledge of MS Office Suites (Outlook, Word, Excel, PowerPoint), as well as ability to use office equipment efficiently (telephone, scanner, photocopier and fax).
Experience and understanding of issues of marginalization and oppression are necessary to work in this role.
Ability to engage in physical activities related to sports, board games, play based activities, etc., and UMAB training.
Ability to work flexible hours.
Ability to travel to various locations, including clients' homes if required.
Valid driver's license and access to a vehicle is an asset.
