Description

Behavioural Supports Ontario (BSO) is a provincial initiative aimed at redesigning the system of care for older adults with complex and responsive behaviours associated with cognitive impairments, mental health and addictions, dementia, or other neurological conditions living in Long Term Care Homes and community settings.

Purpose


The Behavioural Supports Ontario nurse will be the key point of contact within the LTC home focused on residents who are experiencing or at risk of responsive behaviours.


The RN will be responsible to:

  • Provide consultation and behavioural support to individuals, and their families, with various types of dementia, delirium, mental health, addictions and other neurological conditions and those who have experienced acute behavioural change
  • Assist staff in long term care homes to understand and develop effective approaches to provide care to residents who are experiencing or at risk of responsive behaviours
  • Provide consultation, assessment, and care planning for residents who are experiencing or at risk of responsive behaviours
  • Work collaboratively with staff to ensure a supportive environment that enhances resident care
  • Review care plans and recommend changes if appropriate to effectively manage residents who are experiencing or at risk of responsive behaviours
  • Ensure the protection of resident rights and act in an advocacy role for the resident
  • Provide education and support to the staff, residents and families within the longterm care home
  • Work in an integrated manner with the Schedule 1 seniors mental health and addictions mobile teams, other specialist supports, and the BSO LHINwide project team

Qualifications:


  • Current registration in good standing with the College of Nurses of Ontario (RN or RPN)
  • BScN and Certificates in Gerontology and Mental Health would be an asset
  • Current Basic Cardiac Life Support Certification
  • Experience in Long Term Care and knowledge of the Fixing the Long Term Care Act 2021, Ontario Regulation 246/2
  • P.I.E.C.E.S. Training, Crisis Prevention, Gentle Persuasive Approaches, Montessori Methods Training OR other related training
  • Two years recent experience working with individuals with various types of dementia, delirium, mental health, addictions and other neurological conditions and those who are experiencing or at risk of responsive behaviours
  • Demonstrated consultation, assessment, critical thinking and problemsolving skills
  • Strong leadership, coaching and mentoring skills
  • Exceptional interpersonal communication skills, both written and verbal
  • Proficient computer skills and experience with Microsoft Office Software
  • Demonstrated commitment to clientfocused care supported by evidencebased clinical best practices
  • A caring empathetic attitude is crucial
  • Ability to take initiative and work independently as well as a member of a team
  • Experience/familiarity with RAI documentation including assessments, RAPS and care planning
  • Commitment to quality improvement and the ability to facilitate knowledge transfer
  • Current and clear criminal check from a Canadian Police Information Centre, including vulnerable sector check completed within the last six months
