OVERVIEW
This position is a contract position until March 31, 2023 with possible extension.


This new position, specializing in practice as a Social Work Mental Health System Navigator working within the recovery model of care, will support community based Primary Care Practitioners and work closely with consulting psychiatrists at Michael Garron Hospital.

Thise role will be of interest to a Social Worker with recent mental health experience, proficient assessment skills and experience in virtual and telehealth care delivery.


The role requires diverse skills including:

  • Administration of standardized assessments;
  • Systems navigation;
  • Telephone and inperson clinical consultation;
  • The ability to triage for acuity, diagnosis and treatment recommendations;
  • Assessment for acute/crisis needs and knowledge of guided selfhelp, either as the treatment of choice or as a bridge to more appropriate treatment.


The individual will contribute to the development and review of integrated primary care this pilot program, including the provision of appropriate data and model development.


EDUCATION

  • Master of Social Work from an accredited school of social work, or equivalent degree, as recognized by the Ontario College of Registered Social Workers and Social Service Workers (OCSWSSW).

QUALIFICATIONS

  • Minimum two (2) years experience working with an interprofessional team in an acute mental health care setting preferred.
  • Strong knowledge of mental health resources and previous mental health experience.
  • Experience using a range of therapeutic modalities standardized assessment tools that are clientcentred and strengthsbased.
  • Extensive knowledge of / and experience providing services with a Motivational Interviewing, Cognitive Behavioral Therapy focus required.
  • Experience using solutionfocused therapies is an asset.
  • Ability to work independently, seek consultation when needed, as well as be part of a team.
  • Excellent verbal and written communications skills.
  • Excellent interpersonal, time management and organizational skills.
  • Excellent problem solving skills.
  • Research experience /education an asset.
  • Knowledge of policies, legislation, program's and issues related to social welfare institutions and services.
  • Computer literacy required.
  • Keep accurate and timely patient records, using the hospital electronic record (PowerChart).
  • Participate in continued program planning, program evaluation and quality improvement initiatives and assist in presenting results and outcomes at relevant hospital and community events.
  • Demonstrated experience and skill interacting with a broad range of mental health conditions, including serious and persistent mental illness, addictions, trauma and conditions associated with suicidality and /or aggression.
  • Familiarity with marginalized populations, including ethno cultural and religious groups, LGBTQ+.
