Position Summary


Multi Generational Housing & Community Centre is currently looking for a dedicated, motivated, and passionate individual to join our team as a Social Worker.

The Social Worker provides LTC residents and their families with professional guidance and support through the admission process and during their stay.

The Social Worker is an integral part of the multidisciplinary team.


Typical Duties and Responsibilities:

  • Assists LTC residents and family members in utilizing financial, legal, mental health and other community resources to enhance the quality of resident life
  • Promotes resident autonomy in decision making for health care, personal, and financial matters
  • Assists resident and family members to access resources in the areas of substitute decision making, guardianship, trusteeship, Personal Directives and/or Enduring Power of Attorney
  • Contributes to the development, implementation and evaluating of the resident care plan as an interdisciplinary team member. Advocates on behalf of the resident and/or family as appropriate
  • Participates in care conferences and assists in resolving family issues and complaints
  • Completes required documentation in a timely manner
  • Participates and attends committees and meetings as appropriate
  • In collaboration with the Recreation Therapy Lead plans and conducts memorial services
  • Responsible for both the RAI MDS and the Resident Assessment Protocols (RAPs) as it pertains to social work which includes care plans and progress notes
  • Developing and maintaining relationships with the internal and external community

Qualifications:

  • Baccalaureate Degree in Social Work from an accredited university
  • Current registration with the Alberta College of Social Workers
  • Minimum of 3 years' experience as a Social Worker
  • Experience working with the geriatric population in a long term care or supportive living environment will be an asset
  • Experience working with RAI MDS and Point Click Care an asset
  • Ability to communicate fluently in English, both verbally and in writing required.
  • Excellent skills in MS Office suite, and electronic charting systems
  • Knowledge of the Ismaili culture an asset


In an effort to further protect our residents as well as other employees and volunteers, Multi Generational Housing & Community Centre requires all employees to produce a clear Vulnerable Sector Search within 6 months prior to starting work.
