The Home Share Program Coordinator (PC) 1 is responsible for the day‐to‐day operations of VITAL Society contracted services, limited to Home Sharing and Adult Respite Services.


Job Duties

  • Duties allocated will include but not be limited to:_
  • Home Share PC1 recruits, screens and assesses the appropriateness of potential Home Sharing and Respite Providers.
  • Monitors contractors to ensure compliance with contract terms, Provincial Standards for Home Sharing and other requirements.
  • Monitors contractors using vulnerability/risk assessments, health and safety reviews, personal planning meetings, regular visits and other reviews as required.
  • Evaluates contractors formally and informally on an ongoing basis and recommends appropriate training activities and/or support strategies.
  • Provides oversight of and access to personal/confidential information associated with the Person Serviced (financial, health, legal) and ensures the privacy of such information and documentation.
  • Mediates difficult situations and collaborates with all parties to provide remedies to resolve complaints and issues. Models mutual respect and cooperation.
  • Makes recommendations to the Contract Director with regards to contract continuance.
  • Maintains Person-Centered Service Delivery model.
  • Ensures effective service delivery consistent with service standards (Accreditation, CLBC) and VITAL Society mandate, policy, legal, contractual, budgetary and other requirements.
  • Collaborates with service delivery professionals, support workers and other resources who assist and provide various services to the personsserved.
  • Assists families and individuals with planning, advocacy, and act as a support and resource for individuals and families where appropriate.
  • Identifies situations of abuse/neglect and responds with the guidance of the Contract Director in keeping with organizational policies and procedures, in addition to relevant CLBC policies and standards.
  • Facilitates the annual planning meeting of the person served and ensures that the individual's personal network members are invited to participate.
  • Ensures the effectiveness of the Personal Planning process and ensures each Individual's file on Nucleus is up to date.
  • Provides information about community resources to families and contractors.
  • Networks and collaborates with CLBC officials and other professionals in regard to individual participants.
  • Maintains current awareness of programs, specialized services and health care in applicable community living and generic services where they may benefit service participants.
  • Continuous Quality Improvement
  • Develops and monitors standards and accountability/control systems for assigned individuals/area of responsibility. This includes recommending plans and objectives to the Contract Directors.
  • Ensures required records and documentation are maintained and submitted to the appropriate agencies.
  • Ensures that all operational and individual records and documents are maintained and appropriately submitted as per VITAL Society policies and procedures, CLBC guidelines, contractual obligations and accreditation standards.
  • Provides feedback and suggestions to the Contract Director regarding the planning, assessing, and resolving of any operational issues and policies.

Required Qualifications

  • To be knowledgeable and adhere to all care plans
  • Valid Class 5 Driver's license and an acceptable driver's abstract
  • Valid First Aid
  • Obtain Community Living Home Study Certification for Shared Living.
  • Ability to obtain business insurance on vehicle (employer pays difference)
  • Knowledgeable with CLBC Standards of Home Sharing and funding guidelines
  • To be knowledgeable in Health and Safety requirements
  • The use of a reliable personal vehicle with insurance per Vitals' policy

Desired Qualifications

  • Welldeveloped ability to plan, organize and provide communitybased services and supports to youth and adults who are referred to the service, including individuals who may have intellectual, emotional, health/mental health, behavioral, physical and/or addiction support needs and accommodations.
  • A commitment to and knowledge of positive behaviour support principles and strategies.
  • Excellent knowledge of theory, principles and practices related to the community living sector.
  • Ability to provided skilled guidance and oversight to Program Coordinators and Contractors according to VITAL Society governance framework, CLBC, Accreditation Service Standards and VITAL Society policies and procedures and other protocols/practices that may apply.
