Nurse Practitioners will provide care to individuals who need assistance as they deal with the effects of aging, injury, surgery, or illness.
You will use an evidence-based approach to assess, complete a diagnostic work-up, treat, make referrals to specialists and allied health for patients and residents.
Please note that this is a relocation position at a long-term care facility.Help formulate a plan of care based on patient assessment in collaboration with the Care Team, facility, patient/family/caregiver and other health care professionals
Perform advanced, comprehensive health assessments in-person
Evaluate effectiveness of treatment plans and adjust accordingly in consultation with the patient/family/caregiver and other health care professionals
Excellent verbal and written communication skills to effectively interact with clients, colleagues, and other healthcare professionals.
Understand and respect economic, cultural, religious, racial, and gender diversity to provide inclusive and sensitive care to clients from various backgrounds.
Proficiency with computer and phone applicationsExperience in performing advanced, comprehensive health assessments in person or over a telemedicine platform
Excellent problem solving, time management, and crisis resolution skills
Master of Nursing-Nurse Practitioner (MN-NP), Primary Health Nurse Practitioner (PHNCP), Master of Nursing/Primary Health Care Nurse Practitioner (MN/PHCNP), or related Nurse Practitioner Master's Degree
Valid Standard First Aid and Level C CPR
Updated immunization records (including completion of a two-step tuberculosis test within the last 6 months)
Valid G2/G driver's license and access to a reliable car.
