The Role and Opportunity:


The Canadian Mental Health Association is looking for a Case Manager - Regulated Health Professional (Registered Nurse, Social Worker, Occupational Therapist) to join our Forensic Supportive Housing (FSH) Program.

The FSH Program delivers intensive case management and housing support services to individuals living with mental illness who are involved with the forensic criminal justice system.

Reporting to the Manager of Forensic Services, and in collaboration with the FSH Clinical Lead, the Case Manager will provide direct service to clients and consultation to an interdisciplinary team designed to deliver enhanced service intensity and specialized supports to individuals living independently in the community.

The Case Manager will use evidence-based tools to screen, assess and deliver interventions to clients experiencing mental health and addiction challenges, supporting them to remain stably housed and to meet the requirements of the Ontario Review Board.

RESPONSIBILITIES

  • Assessment, service planning and discharge planning
  • Clientcentred engagement, service provision and interventions
  • Providing skill development, linkages, resources, information and advocacy
  • Crisis prevention and intervention
  • Provide psychoeducation around medications and symptoms
  • Assess mental status and monitor physical health needs
  • Housing support and eviction prevention activities
  • Maintaining trusting, respectful and collaborative working relationships
  • Collaborating with the forensic hospital care team to achieve positive outcomes
  • Participating and reporting to Ontario Review Board (ORB) hearings
  • Supporting clients in adhering to their ORB conditions

SKILLS YOU BRING

  • A valid Ontario Driver's License, and access to a vehicle and insurance, are required.

Salary Range:
RN - $57,654 - $71,508;
OT/SW - $56,524 - $70,106;


SSW:
