Transitional Care Assistant-Canadian Red Cross

· Facilitate/arrange the client'stransportation home utilizing local transportation services and/or transportsclients home by driving agency vehicle when needed .

· Plan, arrange, procure, or performhome management duties (cooking, cleaning, laundry, etc.), in accordance withestablished service plan and in advance of discharge when identified.

· Support clients post-discharge toremove barriers and reduce risk in accessing resources (pharmacy blister packs,assistive devices, referrals, etc.) and for follow up discharge-relatedappointments.

What weare looking for:

· Minimumrequirement of completion of two years post-secondary education with aconcentration in social services, gerontology, therapy assistance or a relatedfield, and a minimum of three years job-related experience or an equivalentcombination of education and experience .

· Previous knowledge ofmedical and technical terminology is an asset. Successful candidates withoutworking knowledge of medical and technical terminology will are expected todevelop a working knowledge of medical and technical terminology in the role.

· Validdriver's license and access to a reliable vehicle.

Working conditions:

· Duties are principally conducted at the in theresidencies of the clients, located within Cochrane, ON .

· This position requires a valid provincial driver'slicense as well as access to a reliable vehicle, and First Aid certification(training provided).

A good range of motion is required to assist clients with an arm-assistor pushing a wheelchair.
