**_Program Overview - Supportive Housing, Community Care
- *
  • The St. Hilda's Supportive Housing Program provides housing for seniors who have been street involved, and who may be experiencing mental health and/or addictions issues.
The program provides onsite psychosocial, primary care supports and services including:

intensive case management; harm reduction and addiction supports; therapeutic and supportive counseling; 24-hour staffing; assistance with activities of daily living; nursing services; life skills development; social and recreational activities; volunteer opportunities; and follow up support.

_What You Will Do_
- _

Conduct tenant needs assessments, create care plans for individuals who have current or past hoarding tendencies and/or limitations for unit maintenance and engage tenants in identifying any issues/problem areas inside unit for creating a healthy living environment.

_
- _Encourage and support tenant engagement in the ongoing management of their unit to achieve positive outcomes as related to decluttering, garbage removal, preparation for pest treatments, and collaborate with Housing team and/or exterior contracts for disposal of items from unit._
- _ Coach/educate tenants on their responsibilities as per signed tenancy agreement with WoodGreen Housing and work with tenants to ensure clear understanding of housing concerns as related to fire safety with hoarding and/or participation in pest control treatments. _
- _Identify resources and develop collaborative plans with tenants with respect to issues of hoarding behaviour. _
- _Ensure tenant ownership and empowerment through processes of decluttering units._
- _ Adapt/evaluate decluttering plans and programs on a regular basis while working with tenants to meet changing needs. _
- _Collaborate with support teams, Supervisor, and housing staff to plan and facilitate appropriate pest control treatments. _
- _Help tenants to set/achieve realistic goals with respect to bringing their unit to an acceptable safety standard and help tenants to achieve set goals through supportive means such as encouragement, coaching, psycho-socio education, and de-escalation. _
- _Attend Hoarding Network and community related meetings to network/keep up to date on best practices for hoarding supports, and follow up for behavioural issues that impede progress. _
- _Liaise with Supervisor and team to coordinate information sessions for tenant communities and/or 'one to one' coaching on de-cluttering and maintaining healthy living environments. _
- _Provide crisis intervention and conflict resolution with residents when required._
- _ Monitor/review tenant needs, community issues, and communicate information to Supervisor._
- _ Carry out administrative procedures and maintain appropriate records with respect to pest-control treatments and interventions._
- _ Uphold confidentiality policies regarding tenant information._
- _ Prepare statistical and narrative reports of the interventions for the Supervisor._
- _ Upgrade training and education as directed to fulfill the responsibilities of the job._
- _ Ensure the anti-racism objectives of the organization are achieved within the program._
- _ Represent the unit on internal staff committees as directed by the Supervisor._
- _ Participate in regular supervisory sessions with the Supervisor and staff/unit meetings._

What You Bring to the Team_**- _

Post-Secondary Degree in Social Work or a related field; or Diploma in Social Services with several years of related experience.

_
- _ 2+ years' experience in social services working with individuals who have history of homelessness, low income, addictions, mental health challenges, violence, and/or abuse._
- _ Experience in crisis response, intervention, de-escalation, advocacy, coordinating services, implementation, and negotiation skills._
- _ Experience with client centered, anti-racist, and anti-oppressive frame work._
- _ Knowledge of bed bug and other pest concerns, pest control procedures, solutions and best practices for hoarding support and follow up._
- _ Effective September 7, 2021, all newly hired employees will be required to provide proof that they have completed an approved course of vaccination against COVID-19 at least 14 days prior to commencing employment or provide documentation to support an exemption based on grounds covered by the Ontario Human Rights Act (i.e. Medical exemption)._
