The Client Service Intake Coordinator - Under the direction of the Immediate Supervisor, is responsible for timely data entry of all client referrals and updated information.


DUTIES AND RESPONSIBILITIES:


  • Handle an document prospective client inquiries regarding requests for care and service.
  • Complete data entry and maintain accurate current careplan and scheduling documentation.
  • Handle and monitor discharges and on holds.
  • Maintain filing for all client charts.
  • Organize and supervise all incoming referrals and memos.
  • Receive priority referrals by phone from AHS.
  • Assess information received for errors and notify AHS case manager with any discrepancies.
  • Answer phones and follow up with any priority referrals.
  • Accurately monitor and maintain field staff work alone safety legislation.
  • Adhere to Bayshore Policies and Procedures.
  • Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System.
  • Participates in proactive Health & Safety activities while performing all duties. Is responsible to notify immediate Supervisor of any Health & Safety risks or concerns.
  • Maintains confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel.
  • Complete other tasks as requested.
  • Monday to Friday 9am to 5pm

Job Qualification

Education
Minimum - Secondary School Diploma. Completed of a recognized program such as Medical Office Assistant and/or a Medical Terminology course is preferred.

Experience
Minimum of 2 years related customer service experience, preferably in healthcare.

Other Skills and Abilities
Exceptional organizational and interpersonal skills. Capable of prioritizing a variety of tasks effectively. Proven ability to work both independently and as part of a team. Ability to meet deadlines and strong commitment to continual learning. Demonstrated competency in keyboarding and Windows software. Ability to operate all standard office equipment. Proficiency in written and spoken English.
