Base Rate:
$112,050 - $168, 100 (Salary band placement commensurate with experience)

Reporting to the Program Director, Palliative, Seniors' Services, Primary Care and Health System Integration, and with accountability to the Program Chief and Medical Director of the Program, the Clinical Manager, Seniors Services is an integral member of the Program's leadership team.

The Clinical Manager will work closely with physician leads in each of the service streams within the portfolio to ensure delivery of high-quality services that are effective and sustainable.


The Seniors Services Program at THP is a regional program that encompasses a number of services delivered in the emergency department, inpatient programming, outpatient/outreach programs within the Mississauga Region.

The Clinical Manager will hold responsibility for the programming and operations of a 26-bed Inpatient Seniors' Acute Unit located at the Mississauga Hospital, lead an interprofessional team supporting the GEM, Inpatient Consultative Services, and Outpatient/Outreach services across multiple THP sites and extending into the community.

The Clinical Manager will maintain strong working relationships and engage in a strategic capacity with key partners internal and external to THP, including (but not limited to) Community Outreach Programs, Mississauga-Halton Home and Community Care Support Services, Ontario Health (Central); Mississauga Ontario Health Team, and the local Primary Care community
. In the clinical practice realm, the Clinical Manager has an integral role in supporting the professional practice development needs of these interprofessional teams and provides consultative services and clinical advice to the teams to enable them to deliver the highest quality, evidence informed services.


Key Qualifications:


  • Completion of a baccalaureate degree in a healthrelated discipline
  • Masters degree strongly preferred
  • Exceptional leadership and professional practice skills
years progressive leadership experience

  • Excellent performance and attendance record
  • Previous experience managing regional programs an asset
  • Proven ability to establish positive relationship, team building
  • Proven ability to manage programs/teams across multiple sites and/or multiple programs
  • Effective team building and creative thinking skills along with the ability to function effectively during periods of rapid change and transition
  • Experience with budge management
  • Ability to create a vision for the development of comprehensive programs related to the various services provided now and, in the future,
  • Analytical, problem solving, and effective communication skills are essential
  • Provide support to a patient and familycentered environment
  • Advanced working knowledge of computers and relevant software
  • Demonstrated experience in program planning, implementation and development
  • Valid drivers' license with a clean driving record and a vehicle required

Leadership Responsibilities:


  • Plan, coordinate and effectively manage resources (human, financial, physical)
  • Lead portfolio teams in program development, planning, implementation, evaluation and ongoing improvement
  • Collaborate with external and internal stakeholders in the development and delivery of an integrated and highquality patient care and patient experience
  • Lead the development and sustainability of high performing teams
  • Data analysis of quality, utilization and outcome indicators
  • Maintain linkages and coordinate services with other community providers and regional partners
  • Actively participate in committees, both internal and external, addressing collaboration and coordination of services
  • Develop, plan and implement entrepreneurial and innovative ideas that enhance service delivery and cost effectiveness
  • Support innovation in processes, technology and people and action innovations that improve the quality and/or sustainability of services
