PURPOSE OF POSITION


The Intensive Services and Transition Worker provides evidence based/informed programs to youth and their families in their home, school and community to assist with transition planning and support gains made through Live In Treatment Service (LITS) or community services/programs.


JOB DUTIES AND RESPONSIBILITIES

  • Meet with children, youth and/or families at home, school, in-office and/or in the community to further support treatment gains and transition plans utilizing evidence based/informed programs.
  • Participate in case planning meetings to provide input into the treatment goals and objectives, plans for service, and programs achieving the goals of clients.
  • Develop collaborative client transition plans and implement these plans across all life domains, including out-of-region supports.
  • Deliver evidence based/informed intervention services through individual, family or group meetings.
  • Work in partnership with hospital day program to provide transition support to youth and their families.
  • Work as a part of a multidisciplinary treatment team in delivering client services and act as case manager for assigned clients when required.
  • Perform Crisis Intervention, Medication Administration and First Aid/CPR.
  • Complete centralized intake screenings and presentations.
  • Provide support to LITS program as required.

QUALIFICATIONS

  • Child and Youth Worker Diploma.
  • Minimum 2 years' experience working with children, youth and their families in a children's mental health or related service.
  • Experience working with evidence based/informed programs.
  • Knowledge of youth and family engagement strategies and behavioural interventions for working with children, youth and families.

SKILLS AND ABILITIES

  • Collaboratively approaches problems from a solutions based perspective by reviewing the viable alternatives and determining the best course of action.

  • Inspires and challenges others to achieve personal and organizational goals.
  • Competency and effective use of database and Microsoft Office systems.
  • Confident, articulate and professional when communicating verbally and in writing.
  • Strong case management, assessment and case coordination skills.
  • French language is an asset.
