About the Job:


Reporting to the Dufferin Services Team Program Manager and working in collaboration with the County of Dufferin, the Housing Stability Specialist will be responsible for providing high quality, integrated, person-directed housing support and services within the County of Dufferin to individuals who are on the By-Name-List (BNL) and/or being supported by the County of Dufferin and may be facing eviction.

The Housing Stability Specialist role offers intensive case management within a Housing First approach to support individuals with experiences of homelessness to access and maintain supportive housing.


They will provide information on resources, coordination of services, and general mental health support through a person-centered model of support.

Duties and Responsibilities:


  • Collaborate in developing and implementing individualized recovery plans with clients
  • Use standardized assessment tools designed to improve and/or stabilize client's physical and mental health, foster independence, and promote participation and community integration
  • Provide strength based case management that recognizes clients unique needs
  • Assist with activities of daily living, conflict resolution, and crisis avoidance
  • Support individuals with accessing, maintaining and sustaining housing.
  • Participate in team meetings, consultations, community tables and case conferences as needed to address risks, support solutions, and assist in dispute resolution to support clients
  • Liaise with landlords and housing providers for access to information, education, and support
  • Partner with clients and their families as members of the health care team
  • Develop and maintain a community resource network
  • Collaborate with partnering organizations and provide linkages to community resources
  • Work independently and collaboratively within a multidisciplinary team to carry out mental health recovery and housing support
  • Understand and be able to integrate a philosophy of person and family centered care into daily responsibilities
  • Demonstrate a strong commitment to quality care and service delivery based on SHIP's Mission and Values
  • Ensure appropriate records are kept and ensure that statistical information is maintained as per agency standards
  • Adhere to agency Health & Safety Program and the Client Safety Framework in the promotion of client safety
  • Perform additional duties including providing support to other programs within the team as required/delegated by Management

Qualifications:


  • Postsecondary education in Social Work, Health Services, Psychology or equivalent
  • 3+ years providing Mental Health or Housing case management
  • Demonstrated knowledge of serious mental illness, and/or concurrent disorders
  • Experience working with individuals experiencing homelessness
  • Knowledge of evidencebased practice for mental health services
  • Excellent assessment, risk management, and crisis intervention
  • Expertise in collaboration with healthcare services
  • Knowledge of MS Suite (Word, Excel, Power Point and Outlook)
  • A vehicle, valid driver's license, and adequate insurance coverage
  • Valid Certification in First Aid & CPR
  • Training in ASIST and CPI an asset
  • Experience using HIFIS an asset

Working Conditions and Physical Requirements:


  • Hours are typically from Monday
  • Friday, 8:30 am 4:30 pm but may vary depending on client needs
  • Average amount of physical effort is required.
  • Able to sit/stand for prolonged periods of time
  • Able to answer and communicate telephone calls and general computer use
  • Able to lift up to 20lbs (product, documents, etc.)
  • Vehicle travel to sites for visits/walkthroughs and open doors as required. This position includes providing services across the County of Dufferin as well as attending other locations where training and services are provided as needed.
