Job Summary
The Respite Coordinator is responsible for scheduling and coordinating respite care to take place in client's homes.

They will work with clinical staff or referring partners to implement a plan of care to be carried out by a respite worker.

They will coordinate respite services according to the needs of both the care partner and person living with dementia and will schedule and oversee Respite Workers to ensure the program operates according to policies, procedures, and guidelines.


Essential Duties and Responsibilities

  • Maintain a current knowledge of Alzheimer's disease and related dementias
  • Participate in the recruitment of Respite Workers
  • Support the planning, coordination and delivery of all activities pertaining to the client's care plan, providing support and feedback to respite workers in the program
  • Responsible for scheduling, maintenance, and coordination of all respite program hours
  • Respond to ongoing changes in scheduling, considering clients expressed need for services
  • Conduct intake assessments to assess eligibility for service and establish a plan of care for clients
  • Conduct risk assessments and develop risk management strategies to ensure safe delivery of services
  • Link clients and families to education, information and support as needed
  • Provide education, supervision, and support to all respite staff to ensure consistent, knowledgeable, and professional delivery of services
  • Assist clients and families to navigate supports in the community, address barriers to service, and facilitate transitions for care
  • Foster and maintain positive relationships with all other community partners through advocacy and referral maximizing opportunities for clients and their families
  • Observe and report any changes and/or information pertaining to the wellbeing of clients according to agency's policies and procedures
  • Report any serious changes, injury or occurrences to Clinical Staff and/or the Program Manager according to policy and procedure
  • Provide ongoing communication with Program Manager to consult on client care plans
  • Support and participate in the ongoing evaluation of programming to meet the individual client needs.
  • Schedule and attend staff meeting with respite program support staff and Program Manager as well as other interdisciplinary meetings, as needed
  • Promotes a safe environment for both the client and the respite support worker
  • Maintain client confidentiality and respect client individuality
  • Perform other duties consistent with the job classification, as required

Hours of Work

  • The Respite Coordinator generally completes all duties within normal business hours; however, flexibility in hours is required, with occasional evening and weekend work.
  • Participation in the on call rotation is a requirement of this position.

Job Qualifications

Education:


  • Minimum Registered Practical Nursing Diploma with current registration with the College of Nurses of Ontario

Experience:


  • Experience working with vulnerable populations in a community or group setting
  • Experience coordinating and scheduling work flow

Other Knowledge, Skills, Abilities or Certifications:

  • Knowledge of Alzheimer's disease and other dementias
  • Ability to work independently and as a cooperative team member
  • Analytical skills to assess client's needs, functional level and ability to perform Activities of Daily
Living and Instrumental Activities of Daily Living

  • Excellent organizational and decisionmaking/problemsolving skills
  • Time management skills to follow an established schedule
  • Interpersonal skills and the ability to communicate with clients, colleagues, supervisors, agency personnel and other health care professionals.
  • Mindfulness of cultural sensitivity and diversity
  • Ability to speak French or other languages an asset

Travel Requirements:


  • The duties of this position do not require travel other than for occasional meetings held off site, or for travel to workshops and other opportunities for professional development

Physical Demands:


  • Performance of general office duties
  • Significant periods sitting at the computer, but with the opportunity to move away from the work station
  • Sensory attention is required for the majority of the work day (looking at computer screen, reading documents, etc.)

$45,073.00-$67,610.00 per year
