***: This Social Work position will work within an interdisciplinary team in the Trans Health program. They will work with peers, nurses, physicians, medical assistants, lab staff, and a psychologist to provide client centred guidance and support to individuals accessing gender care at Klinic. The social worker intakes clients into the program and remains connected as necessary for counselling support, system navigation, name changes and other episodic issues. They will uphold Klinic's vision, mission and values while promoting social change, problem-solving in human relationships, and the empowerment and liberation of people to enhance well-being.


QUALIFICATIONS:


  • Bachelor of Social Work degree
  • Current registration with the Manitoba College of Social Workers
  • Minimum of two years supervised experience in community health or related area
  • Experience in program and community development
  • Ability to work in an interprofessional environment
  • Ability to work from a harm reduction model.
  • Strong understanding of gender diversity and trans healthcare practices.
  • Understanding the social determinants of health and their impact
  • Demonstrated commitment to communitybased health care
  • Knowledge of community resources
  • Experience providing crisis counselling including risk assessments
  • Experience facilitating groups
  • Experience supporting social work student placements
  • Awareness of and sensitive to issues of cultural diversity
  • Ability to work from an antioppression and antiracism perspective
  • Demonstrated effective communications skills
  • Excellent interpersonal skills
  • Ability to work flexible hours, including evenings and weekends
  • Ability to work independently and demonstrated decision making abilities
  • Proven ability to work from an empowerment model
  • Proven ability to optimize team and program functioning
  • Knowledge of primary health care

RESPONSIBILITIES:


  • Provide social work services within the parameters of the Manitoba College of Social Workers Standards of Practice and Code of Ethics
  • Conducts Trans health services client intake
  • Provide short term supportive counselling
  • Provide crisis support when a referral to the crisis lines is not appropriate
  • Assist clients navigate formal social service systems such as EIA
  • Provide support, information, referral and links to appropriate community resources
  • Liaise with other programs at Klinic for health services
  • Engage in building and supporting an interprofessional environment
  • Liaise with community resources/networks and service organizations
  • Supporting Social Work student placements as a Field Instructor
  • Facilitates groups
  • Provide public education and training activities
  • Provides support for learners
  • Works as part of a team and promote team functioning
  • Charts appropriately according to Klinic procedures
  • Participates in training courses and in services offered elsewhere and by Klinic to maintain and improve quality of service
  • Attend staff meetings as required
  • Participates in program planning, evaluation and quality monitoring including completing daily statistics
  • Participate in formal self evaluations by identifying strengths and limitations
  • Seek appropriate supervision/consultation of own actions
  • Adheres to all agency policies, protocols, procedures and promotes best practice
  • Other duties and functions related to their job description not exceeding the above qualifications

SALARY RANGE $29.867 per hour - $38.977 per hour
