Job DescriptionKEY ACCOUNTABILITIES
- Personal Care
care plan.
Activities:
- Assist client/patient with bathing: shower, sponge bath, tub bath and bed bath.
- Makes beds occupied/unoccupied.
- Assists client/patient with grooming.
- Provides and maintains good skin care.
- Observes and reports changes in skin condition.
- Provides partial/total assistance with dressing.
- Toilets client/patient:
oSupports individual's usual elimination pattern and habits
oRecognizes change in usual frequency, character and/ or pattern of elimination and reports as required
- Implements simple measures to encourage elimination (e.g., extra fluids, running tap, etc.)
- Uses toileting aids/continence products:
oUtilizes bedpans, urinals, commodes, incontinence products
- Cleanses perianal area.
- Measures and records fluid intake and output.
- Implements activation/restorative care programs previously determined by care plan.
- Encourages and assists with client/patient participation in planned activities and own care.
- Encourages exercise including active rangeofmotion according to care plan
- Incorporates family preferences and routines.
- Lifts and Transfers
- Utilizes proper body mechanics and transfer techniques to carry out safe lifts and/or transfers.
Activities:
- Uses safe and effective body mechanics.
- Assists client/patient to transfer in a way that maintains and/or enhances ability.
- Transfers client/patient with assistance of one person.
- Assists client/patient with mobility:
oone/two-person lifts
ocrutch-walking
ocane
owalker
owheelchair
oDemonstrates safe methods of repositioning client/patient in bed/chair.
oRecognizes, declines and reports unsafe ambulation/mobilization/ transferring equipment.
oReports changes in mobility.
- Nutrition/Food/Storage
- Prepares nutritious, varied and economical meals and snacks, including special diets as determined by the care plan,and uses proper food preparation, storage and hygiene practices.
Activities:
- Works with client/patient to identify client/patient's desired level of involvement in meal preparation;appropriately negotiates issues about meal preparation.
- Supports and assists client/patient with shopping.
- Demonstrates appropriate meal planning and food selection with client/patient involvement.
- Plans meals with sensitivity to client/patient's budget, culture, religion, social needs and any special dietaryrestrictions if needed.
- Prepares nutritious and attractive meals and snacks using proper cooking techniques.
- Seeks direction in determining degree and appropriateness of assistance with feeding of client/patient with partialor total assistance.
- Encourages enjoyable food and fluid intake.
- Observes food and fluid intake and reports changes.
- Safely and hygienically stores and handles food.
- Demonstrates the use of special adaptive devices for eating/drinking
- Cognitive, Social and/or Emotional Support
Activities:
- Provides orienting information as required by the individual.
- Encourages client/patient in decisionmaking process.
- Implements simple measures to minimize aggressive behavior.
- Implements care plan previously determined by health care professional(s) to prevent/handle aggressive and other problematic behaviors.
- Develops supportive relationship with client/patient.
- Promotes independence and selfcare.
- Assists client/patient to maintain an acceptable lifestyle.
- Recognizes importance of observing behaviors and reports any changes.
- Adaptive Aids
Activities:
- Hearing devices/aids.
- Glasses, contact lenses.
- Dentures (full and partial plates).
- Home Management
Activities:
- Uses appropriate cleaning techniques (e.g., kitchen, bathroom, bedroom, etc.).
- Proper and environmentally friendly use of household cleaning products.
- Launders clothes appropriately.
- Properly operates and cares for standard household and cooking equipment.
- Identifies types and uses of client/patient's personal equipment.
- Medication
Activities:
- Supports client/patient's independence regarding medication.
- Recognizes and reports changes in a client/patient's behavior/condition.
