    Currently, our team is looking for a Diabetes Educator (Registered Dietitian or Registered Nurse) who can work with us on a temporary (18-month contract) part-time (2 days per week) basis at our Vaughan location. Please note that this is a hybrid position based on clinic needs.

    Responsibilities
  • Educate and advise patients on self-management skills in both individual and group settings focusing primarily on diabetes and secondarily on other health conditions such as general health & metabolic conditions (weight management & obesity).
  • Develop a care plan for patients and collaborate with the multidisciplinary team, as needed.
  • Complete detailed patient encounters within EMR system.
  • Teach administration of insulin effectively, including titration and management, as per medical directives (for applicable patients).
  • Teach self-monitoring of blood glucose (SMBG) effectively for patients living with diabetes, as per medical directives.
  • Perform patient follow ups in person or remotely (email, telephone, video).
  • Obtain relevant health history of patients.
  • Provide consultation as requested to community groups, partners, and team members.
  • Training of additional staff and any new educators, as required.
  • Effectively communicate with patients, families, caregivers, and healthcare professionals.
  • Employ information technology appropriately for patient care (i.e., statistical tracking).
  • Stay current and aware of opportunities to implement new, evidenced-based methods of patient assessment and treatment.
  • Assist with research studies as needed including providing diet counseling for those patients enrolled in research studies.
  • Complete a minimum of 2 community outreach events annually.
  • Complete annual outreach calls.
  • Assist with program implementation which includes reviewing patient charts, entering data, & following program guidelines.
  • Responsible for administrative tasks (can be delegated to administrative staff/volunteers/interns as needed). These responsibilities include but are not limited to: rescheduling follow-up appointments, creating workshop folders, ordering supplies & medications, tracking patient visits, & entering data for programs.
  • Participate in Journal Clubs & other group learning opportunities.
  • Participate in disseminating the program vision.
  • Participate and promote non-ministry funded LMC initiatives, including paid and non-paid services.

  • Education/Experience
  • A valid RD or RN license in good standing with the College of Dietitians of Ontario or College of Nurses of Ontario
  • Experience teaching/counseling individuals and groups of people.
  • Must obtain or be working towards CDE designation.
  • Must have a valid driver's license in good standing and access to a personal vehicle.
  • Experience in motivational interviewing and other evidence-based coaching techniques an asset.
  • Health coaching experience is welcomed, or open to training as a health coach.
