The Opportunity:


Reporting to the Managers of Rehabilitation Services, this Social Worker practices as a member of interprofessional teams to provide services to children and their families within the rehabilitation portfolio.

This position will provide service primarily to ambulatory care areas including Developmental Clinic and Neurology, but may also support other areas within Rehabilitation Services such as inpatient rehab care team and other rehab teams.

The Rehabilitation Social Worker supports children and families to build their capacity to identify and engage in accomplishing their goals; provides services and advocacy within the health care setting and in the community; provides psychosocial and family system assessment; provides individual and family therapy; supports families in navigating and accessing services within the health center and the community; and provides information and referrals to other team members and services.

The Social Worker builds professional relationships with collaborating agencies and community partners; participates in collaborative center-based and community-based committees and initiatives to support and evaluate the services offered to individuals; participates in service development/improvement; and encourages an inclusive model of care.

The role of the Rehabilitation Social Worker includes being on a rotation schedule to provide intake services for the Children's Health Program (1-2 times a month).

The Rehabilitation Social Worker also provides occasional backup for service provision to other areas. The Social Worker is required to participate in the rotational weekend and holiday stand-by schedule.

Your Qualifications:


  • Master's Degree in Social Work required.
  • Currently licensed with, or eligible to be licensed with, the Nova Scotia College of Social Workers (NSCSW) required.
  • Minimum two (2) years of clinical experience as a Master's prepared Social Worker required. Clinical pediatric experience preferred.
  • Demonstrated knowledge of the Child and Family Services Act required.
  • Clinical experience (including knowledge of best practices) in the assessment and intervention of children with developmental and neurological disabilities and their families required. This includes having expertise in understanding child development, life span development, and family dynamics in the context of developmental and neurological disabilities.
  • Clinical experience in individual, family, and group therapy is required; pediatric experience is preferred
  • Demonstrated ability to triage care and identify solutions in a fastpaced environment, and manage stressful situations required.
  • Demonstrated Social Determinants of Healthbased practices.
  • Demonstrated knowledge and ability to access healthcare and community resources required.
  • Demonstrated organizational, time management, and communication skills (both verbal and written) required.
  • Demonstrated ability to work in an interprofessional team and independently required.
  • Demonstrated understanding and commitment to Family Centred Care required.
  • Demonstrated knowledge of personinenvironment practice required. Experience in solutionfocused coaching and brief singlesession therapy preferred.
  • Demonstrated ability to develop and maintain professional relationships with the team, collaborating agencies, and community partners required.
  • Valid mínimal Class 5 driver's license and access to reliable transportation for home visits required.
  • Experience in crisis intervention and trauma debriefing preferred.
  • Experience in case management and community development preferred.
  • Competencies in other languages are an asset.
