    Clinical Dietitian II - Regional, Family Care Team, Primary Health Care and Community Health Nursing, Permanent Full-Time, Brookfield/ Centreville FCT

    The philosophy of a Family Care Team (FCT) is to deliver care that is patient-centered, comprehensive, team-based, culturally safe, coordinated, accessible, and focused on quality and safety. This philosophy supports providers and care teams to meet clients where they are using an inter-professional collaborative approach where care is provided in the right place, at the right time, by the right provider in a manner that best suits the client's need. The team will ensure clients are treated with respect, dignity, and compassion, and enable strong and trusting relationships with providers and staff. FCT's will enable the delivery of a range of quality care for clients within their own communities while facilitating interdisciplinary work amongst healthcare providers. The FCT for the Brookfield/ Centreville will include the geographical areas of Brookfield and Centreville.

    Definition of work:
    The Primary Health Care Program is seeking an advanced professional Clinical Dietitian II for the Brookfield/ Centreville FCT. The Clinical Dietitian II is responsible for the provision of nutrition care (i.e. screening, assessment, planning, implementation and evaluation) to clients of the FCT as well as the planning, implementation, coordination and evaluation of nutrition services. The principles of the FCT align with the patient health home model of care; using a collaborative health care team approach, from various health disciplines, in the provision of primary health care services to clients.

    This position functions with a high degree of independence and initiative. The successful incumbent will report to the Regional Manager of the Exploits Family Care Team, Primary Health Care Program and will maintain accountability to the Regional Professional Practice Consultant for Dietitians for integrity and professional practice issues.

    Nutrition services will include (but is not limited to) health promotion, disease prevention, nutrition counselling (group and individual) and chronic disease management. In this role, the Dietitian will support the development, implementation and evaluation of nutrition practice, program and quality improvement initiatives within the FCT; participate in evidence informed practice, education and community capacity building; and play a lead role in the development of primary health care initiatives using a nutrition lens.

    Clients in this primary health care setting have diverse nutritional needs and may include frail elderly, clients with acute and/or chronic disease and those at risk for malnutrition; as such the dietitians practice will reflect the needs of the clients/community. Care will be provided face to face and virtually as necessary and appropriate. The dietitian will apply their skills of advocate and change agent to support the FCT's mission; collaborate with the FCT team in the provision of care; engage with acute care and community supports in the provision of care; assist clients with navigation through the health system; and mentor other dietitians and team members.

    The successful candidate must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centered care, demonstrate a commitment to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements. Travel throughout the region may be required.

    Required qualifications:
    * Successful completion of Bachelor's Degree specializing in Applied Human Nutrition or equivalent field of science as approved by the NLCD, supplemented by a 1-year internship and a Registered Dietitian (RD) professional designation.
    * Registration with the Newfoundland and Labrador College of Dietitians is required.
    * Two (2) years' experience within the last five (5) as a Clinical Dietitian.
    * Thorough knowledge of nutritional health promotion strategies; nutritional interventions to prevent and manage acute and chronic disease; clinical nutrition care processes; behavior change theory; primary care nutrition risk screening strategies and pathways; clinical nutrition presentations requiring referral to secondary health care services; and the broader health system and community supports.
    * Demonstrated ability to communicate effectively with inter-professional team members, patients/residents/clients and their families in a one-to-one and group environment.
    * Demonstrated effective written communication skills required for client documentation, report writing and professional communication
    * Demonstrated ability to assume professional responsibility for safe, ethical and effective practice.
    * Demonstrated supervisory and leadership skills.
    * Demonstrated ability to function effectively in a diverse and changing environment.
    * Demonstrated ability to lead practice initiatives within the nutrition/dietetics community of practice.
    * Consistent demonstration of the core values of NL Health Services: Innovation, Compassion, Accountability, Respect, and Excellence (I CARE)
    * A valid Newfoundland and Labrador Class IV Driver's License.
    * A satisfactory record of work performance and attendance is required for this position.
    * A satisfactory certificate of conduct from the RCMP or local police authority.

    Hours of work: 75 hours biweekly (Shift work may be required)
    Salary: CG-42 ($41.15-$48.48)
