    DUTIES &


    RESPONSIBILITIES:

    Reporting to the Patient Care Manager, GAIN, the Pharmacist will be a member of the interprofessional team and will provide specialized comprehensive geriatric services within the Geriatric Assessment and Intervention Network (GAIN) and geriatric care throughout Lakeridge Health.

    The pharmacist will participate in Comprehensive Geriatric Assessment as the standard of care for frail seniors referred through the Emergency Department, post hospitalization, primary care providers and other community partners.

    The Pharmacist will provide senior-friendly and patient-centered care to geriatric patients by: managing the use of medications and the drug distribution system, educating patients and health care providers, and engaging in clinical trials and pharmacy practice research; using expert clinical judgement, evidence-informed practices and tools to gather, synthesize and interpret information required to understand the patient's story and biopsychosocial needs; utilizing expert geriatric knowledge while applying the pharmacist clinical lens to engage with the interprofessional team, patient, and caregiver to develop a goal based plan of care; providing recommendations designed to maximize safety and function to support frail seniors living at home; providing support to other geriatric teams and units throughout Lakeridge Health as required, and, participating in quality improvement initiatives, attending regular team meetings and care conferences as required, and performing other duties as assigned.

    Travel between sites may be required in order to support all geriatric teams throughout Lakeridge Health.


    QUALIFICATIONS:

    • Bachelor of Science degree in Pharmacy;
    • Current registration in good standing with the Ontario College of Pharmacists, or eligible for licensure;
    • Completion of the Board Certified Geriatric Pharmacist (BCGP) program is preferred;
    • Previous clinical experience with geriatric clients;
    • Ability to complete comprehensive geriatric assessments;
    • Demonstrated knowledge of medical/biological issues as they relate to the aging process (knowledge of delirium, risk factors, medications etc.);
    • Practical knowledge of pharmacology, current best practice clinical guidelines, and ability to interpret lab values;
    **Certificate of completion in PIECES, Montessori Based Dementia Care and GPA;
    *Advanced understanding of dementia and the ability to assess the unique impact of the disease
    *Thorough understanding of the concept of "Responsive Behaviours" and behavioural expression as a form of communication
    *Thorough knowledge of community resources in order to provide linkage to the appropriate services necessary to best serve the client;

    • Strong clinical assessment skills in identifying and solving drug related problems;
    • Excellent medication counseling skills;
    • Demonstrated ability to research and provide drug information effectively to patients and other members of the multidisciplinary team;
    • Effective communication and interpersonal skills;
    • Good organizational skills and ability to prioritize tasks;
    • Proven ability to collaborative and problem-solve;
    • Demonstrated ability to work well with patients, families, all levels of staff and the community
    • Exposure to Lean principles, an understanding of methodologies and tools and a commitment to the use of these in support of providing the best healthcare experience possible to our patients

    • Must have a satisfactory performance record;
    • Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff safety, patient safety and environmental safety, participation in regular in-service sessions; and
    • Proven good attendance record with capability of maintaining this same standard.

    CONDITIONS OF EMPLOYMENT:

    The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and OPSEU Local 348.

    Successful External applicants must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months.
