Temporary Full-time

Under the direction of the Manager of System Performance, the Youth Wellness Hub (YWH) Care Coordinator will perform a variety of administrative, coordination and logístical services to lead the operation of the YWH, and will supervise a team of Peer Support Workers.


DUTIES

  • Administrative & Operational
  • Provide training on different aspects of the YWH to staff, volunteers and community partners.
  • Participate in planning, budgeting and service delivery development of the YWH.
  • Maintain all data services linked with the day to day operations of the YWH such as My Wellness Passport and submit statistical data to funders.
  • Liaise with community partners to coordinate service delivery and scheduling within the Hub
  • Support the creation of a developmentally appropriate, nonjudgemental, nonstigmatizing, safe and welcoming YWH atmosphere.
  • Coordinate projects related to the YWH with support from internal and external stakeholders.
  • Organize and schedule meetings.
  • Develop, collect and maintain accurate financial records in conjunction with the finance department.
  • Maintain inventory of supplies required for Hub programming.
  • Plan, facilitate, participate and contribute to team huddles/meetings/trainings.
  • Maintain and update all required program forms.
  • Represent the Youth Wellness Hub in community initiatives, tables and planning at a local and provincial levels.
  • Provide feedback from regular review of service operations, documentation, and information procedures and systems.
  • Participate in the screening, interviews and selection of staff.
  • Responsible for the supervision of staff and completing staff schedules.
  • Maintain a calendar of events and services for promotion to the general public and partners.
  • Facilitate continued engagement of youth and continuity of care between service providers.
  • Work from a harm reduction, collaborative/teambased and youthcentred approach.
  • Build and maintain relationships with network partners and relevant service providers.

Supervisory:


  • Ensure services offered are consistent with, and reflective of Compass and YWHO's vision, mission, valuesand service delivery principles.
  • Provide staff with the guidance, training, supervision and consultation required.
  • Ensure staff are aware of policies and guidelines and assume responsibility for the integration of such.
  • Responsible for employee performance appraisals as per Compass policy.
  • Promote and ensure that interdisciplinary/interagency cooperation and consultation required is an integral part of case management practices.
  • Responsible for the fair and ethical treatment of staff.

QUALIFICATIONS
- university degree in psychology, social work, business or health administration or equivalent education and experience
- registration with a professional college is considered an asset
- conflict resolution and crisis intervention skills are required
- high level of responsibility, maturity, interpersonal skills and professionalism
- experience working with youth aged between 12-2- oral and written proficiency in the English language is required and fluency in French is an asset
- possession of a valid Ontario Class G driver's licence and availability of an automobile in good working condition
- satisfactory police vulnerable sector check required
