    Job Description

    Accountabilities:

    • Work collaboratively with the Pharmacy Team to provide patient-centred care
    • Provide Pharmaceutical care and be part of a multi -disciplinary team, including medication reconciliation upon admission and discharge, therapeutic drug monitoring
    • Provide drug information to physicians, nurses, and other clinicians to enhance patient care
    • Verification of all medication orders for inpatients
    • Provide education to patients to promote optimal drug use
    • Mentor Pharmacy residents, students and interns
    • Participate on hospital committees as required
    • Dispensary duties including medication order entry and medication profile review
    • Perform and/or supervise in patient dispensing (which includes computerized order entry, unit dose, centralized IV admixture services, TPN, Chemotherapy and an understanding of automated systems) drug information and administrative activities as assigned
    • Resolve day-to-day clinical issues
    • Assist with the implementation of pharmacy initiatives, which improve patient care and other initiatives which improve the efficiency and effectiveness of the Pharmacy operations
    • Other duties as assigned
    Qualifications
    • Currently licensed (or eligible) by the Ontario College of Pharmacists, Part A of the registry
    • Relevant clinical experience preferable
    • Completion of hospital pharmacy residency or equivalent experience an asset
    • Excellent attendance, punctuality and work record
    • Demonstrate initiative and a willingness to work collaboratively in a multidisciplinary team environment
    • Motivated, self-directed learner
    • Strong clinical assessment skills in identifying and resolving drug-related problems
    • Demonstrated ability to apply clinical judgment in analyzing and solving clinical problems
    • May be required to travel between and work at all Osler sites
    • Excellent oral, interpersonal, and written skills
    • Excellent computer skills
    • Must be able to meet the physical demands of the position

    Hours: Days/Evenings, including weekends and statutory holidays (subject to change in accordance with operational requirements)
