    Description

    Salary:

    The suitable candidate will be able to perform the following duties:

    • Conduct comprehensive assessment of referrals for participants treatment, triage referrals based on treatment appropriateness, and communicate effectively with referral agents and participants.
    • Conduct pre-screening of all referrals, establish a waitlist, complete initial intakes, provide referrals as necessary and thorough assessment of participants.
    • Provide consultation services to other agencies and community groups on treatment-related questions for the Woodland Wellness Centre.
    • Develop intake packages for referral agents and participants, ensuring all necessary information is provided prior to intake dates.
    • Utilize traditional practices to address mental health and addiction issues, and improve overall wellbeing of participants.
    • Provide follow-up letters to referral agencies for participants upon treatment completion.
    • Generate month-end reports and annual reports as required to track and evaluate intake processes and outcomes.
    • Maintain collaborative relationships with healthcare professionals, service providers, Provincial health representatives of Aboriginal organizations, and referral agents to assess community health needs.
    • Participate in regular meetings and maintain communication with WWC staff to share information on community needs, program changes and resource utilization.
    • Maintain professional confidentiality within the limits of legal and child welfare reporting requirements and maintain liaison with Band Administration, community and other related agencies.
    • Perform any other task as assigned by the P & R Manager /Director.

    The suitable candidate will possess the following qualifications:

    • High School Diploma plus one to two years college certificate (30 Credit Hours) and/or Diploma in Social Work or other health related field (60 Credit Hours)
    • Administration assistant plus 2 years of experience in working with the public.
    • 2 years of experience in providing intake and assessment are preferred.
    • Combination of qualification and experience shall be considered.
    • Training/Knowledge of Trauma-informed care approaches is preferred.
    • Knowledge of SASSI, MAST and DAST screenings would be an asset.
    • Ability to travel to outlying LLRIB communities, must have a valid Class 5 Driver's License.
    • Experience working with First Nations communities would be an asset.
    • Ability to work collaboratively with diverse teams and stakeholders.
    • Ability to speak woodland Cree would be an asset.
    • Must provide a satisfactory & recent; Criminal & Vulnerable Sector Check, SGI Driver's Abstract.
