Role Description:


Cota's TAY Program provides case management services to adults with developmental disabilities who were formerly in the care of a Toronto-based child welfare agency and who currently reside in a per diem residence.

The Case Manager ensures that clients receive quality support while they are transitionally housed in a per diem residence and actively coordinates placements and transitional planning to move to new settings within the Adult Developmental Services Sector.


Summary of Duties:

Responsibilities of this position include but are not limited to those below:

  • Providing case management to clients in the developmental sector residing in congregate care or host family homes
  • Providing advocacy on behalf of the client and connecting the client to services
  • Ensuring that residential agencies are providing proper quality of care as outlined by QAM legislation
  • Assisting the client in finding meaningful activities (e.g. recreational, educational, employment, etc.)
  • Assisting the client in developing goals and identifying barriers
  • Developing a collaborative, supportive and therapeutic relationship with each
designated client for whom he/she is assigned primary responsibility

  • Completing a comprehensive assessment of the nature of each client's unique
goals and needs through collaborative engagement with each client and their respective support networks

  • Providing guidance and assistance to each client in relation to the development
of both a personalized Client Safety Plan and a Recovery/Goal Plan

  • Assisting each client in accessing and coordinating other supports, services and
Appointments

  • Assisting clients with emergency and/or crisis needs as required/appropriate

Summary of Qualifications:


  • A degree of a recognized university or a community college diploma in a discipline of study in the human services field and/or a recognized program in health/social rehabilitation services
  • Two to three years of experience supporting people with developmental disabilities or other marginalised groups
  • Sound clinical knowledge and understanding of the client population and demonstrated ability to work effectively with individuals with a developmental disability
  • Previous work experience and/or knowledge about how to provide the following services: assessment, service coordination, transition planning, informal counselling and advocacy/rights protection
  • Knowledge of the principles of case management theory/practice using a clientcentred philosophy
  • Excellent knowledge of community resources specific to people living with a developmental disability and demonstrated ability to negotiate with others on behalf of clients
  • Experience with and demonstrated ability to engage and connect with diverse clients and community partners
  • Demonstrated ability to use problemsolving techniques to identify and assess client needs and support client goals
  • Excellent administrative skills, organizational skills, communication skills and interpersonal skills
