Qualifications:


  • Completion of Post-Secondary Education (P.S.W., / Health Care AID) and/or 2 years of the related experience
  • Valid G or G2 Driver's License required

Responsibilities:


  • Performs cleaning of the home on a regular basis, as well as other assigned duties, as per the program requirements.
  • Occasionally facilitates and assists individuals within their Personal Support Plans to achieve identified goals with the involvement of the members.
  • Facilitates essential communications with Primary Direct Care Staff and other services systems as necessary, documenting information in the established communication media.
  • Reports immediately to the Supervisor and/or Program Director of any allegations, or suspicion of abuse, in relations to VITA members, as per VITA's anti-abuse Policies and Procedures.
  • Administers and records prescribed medication as per instructions from medical practitioner and follows the agency's Medication Policies and Procedures.
  • Assists members with personal care and hygiene as necessary. This can include assistance with personal hygiene, feeding, dressing and position change, as well as care, maintenance and use of any equipment required for the support of members.
  • Transports and/or facilitate transportation for our members as required.
  • Identifies and reports equipment and residential maintenance needs.
  • Follows on-call procedures according to Policies and Procedures as needed.
  • Performs other related duties as assigned by the Program Supervisor or delegate.
