    Founded in 2008, House Calls is a pioneering home-based primary care program committed to delivering exceptional healthcare services to frail, home-bound seniors in Toronto. As an independent program, we operate within a collaborative framework alongside SPRINT Senior Care, who takes the administrative lead, and VHA Home HealthCare, who provides contracted Nurse Practitioner and allied health support. Our goal is to support frail, homebound seniors living at home for as long as possible, providing comprehensive and dignified care that includes palliative services. The successful candidate will be joining a cohesive and supportive team that has been providing excellent care in the community for over 17 years

    What will be your role?

    As an active member of the House Calls team, Occupational Therapy Assistant/Physiotherapy Assistant will provide client-centered clinical care under the direction and supervision of a Regulated Health Professional (Occupational Therapist/Physiotherapist).

    As a Occupational Therapy Assistant/Physiotherapy Assistant you will assist/supervise the client with his/her individual prescribed exercise programs or ADL training. Assist/supervise client with walking, transfers, stairs, safe mobility, and positioning using proper body mechanics. Provide client/family education and support to address physical, functional and self-care needs as assigned. Assist with gross motor, fine motor and balance activities. Monitor and report changes in health status to the Physiotherapist and Occupational Therapist. Participate in program planning, quality improvement and evaluation. Document in the client record and collect required data as requested. Supports safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.

    Required Qualifications:

    • Diploma as an OTA/PTA from an accredited college program.
    • 1-2 years of work experience preferably in the community, working with geriatric clients with variable diagnoses and multiple co-morbidities.
    • New graduates welcome to apply.
    • You are a skilled communicator and have a successful track record of coaching/mentoring clients to participate in a professional manner.
    • Current First Aid Certificate and Cardiopulmonary Resuscitation (CPR) Certificate an asset.
    • Evidence of continuous education.
    • Sound critical thinking, good judgement, and problem-solving skills.
    • The ability to work effectively and collaboratively with multi-disciplinary team.
    • Demonstrated understanding and practice of rehabilitation principles and techniques.
    • Demonstrated ability to recognize safety issues and follow safe work practices.
    • Demonstrated commitment to client and family centered care.
