The Long-Term Care Informatics Coordinator is responsible for supporting the implementation of the RAI-MDS and HOBIC projects for the Cardinal Ambrozic Houses of Providence ("the Houses").

The Long-Term Care Informatics Coordinator provides staff training on the use of the MED e-care/RAI-MDS and HOBIC software for long-term care, participates in the evaluation and monitoring of the RAI-MDS and HOBIC projects, tracks and monitors staff progress with completing the required RAI-MDS and HOBIC electronic documentation, and provides recommendations on staff educational needs and performance improvement related to the RAI-MDS and HOBIC projects.


MAJOR RESPONSIBILITIES:


  • As directed by the Clinical informatics Specialist, complete assigned residents Assessments including MDS RAPS and Care Plans according to the timelines set out by the MOHLTC.
  • Under the direction of the LTC Informatics Specialist, and in partnership with the Houses interprofessional care team monitor assignments to ensure timelines are met.
  • Report gaps in work processes and/or issues to the LTC Clinical Informatics Specialist.
  • Provide support to the LTC Clinical Informatics Specialist.
  • Maintain flexibility in scheduling work hours to meet the needs of the role.
  • Complete 1 required module in AIS testing annually.
  • As directed by the LTC informatics specialist attend and if required, facilitate admission meeting. Set up charts in the PCC system, maintain POC tasks for the resident assistant and set up restorative care programs, if applicable for the resident.
QUALIFICATIONS/SKILLS REQUIRED:


  • Registered Practical Nurse with a current Certificate of Registration from the College of Nurses of Ontario.
  • Current AIS certificate
  • Minimum three (3) years¿ experience in Minimum Data Set coding, RAPS, outcome scales, RUGs and care planning in Long Term Care
  • Excellent skills in nursing assessments, communication at all levels, and documentation
  • Excellent understanding of the Long Term Care Regulations and Act
  • Excellent accuracy and attention to detail
  • Ability to work independently (with mínimal direction)
  • Excellent computer skills (experience with PCC, POC, excel, word, outlook, etc.)
  • Demonstrates therapeutic relationship skills and excellent conversations skills
  • Excellent time management skills
  • Ability to meet time lines
  • Very good attendance record;
  • Good performance record.
