Purpose of Job:

the coordinator of support services organizes and oversees in home and out of home supports for foster families and children.

As a result, this person needs to be personable and amiable, as well as efficient and knowledgeable.

Primary Responsibilities:


SUPPORT SERVICES:


Scheduling
- Scheduling of all CYSW is the primary focus within this role. In conjunction with your direct supervisor and Case Managers, you will ensure that all scheduled shifts have a CYSW assigned.

  • Respond to all requests for immediate CYSW support in homes.
  • Assign a therapeutic match for CYSW and clients to ensure our highest quality of care.
  • Communication with supervisor for trouble shooting and support.
  • Call CYSW if end of shift report is not complete.
  • Contact CYSW regarding incident the night before or on weekends
  • Call all weekend staff Monday morning to check in from weekend. Review with them expectations and ensure they set up for their upcoming weekend shifts.
  • Connect with all back up staff weekly, confirm again that they are prepared for the upcoming weekend shifts.
  • Confirming weekend shifts will be done no later then Thursday of every week. We will never go into a Friday not knowing if a CYSW has confirmed they will work their shift.

WE App
- Our online data base is home to all CYSW and client information. This hub will be used daily by yourself and will need to be checked, updated, and reviewed constantly.

  • Check WE Schedule every morning at 9am.
  • Update CYSW (Activities spent in 1 to 1 time) SRA sign off time sheet.
  • Review SRA staff notes for errors, condense lengthiness, and edit out personal information such as Bio family names, shift logs, etc.
  • Confirm safety plans are up to date and complete. Leave no forms blank always add details.
  • Make note that shift report is being followed up (Do not change font from BLUE to RED).
  • Populate CYSW schedule on a 24week cycle. This will be determined based on a foster home or staff run home.
  • Ensure week schedules are PUBLISHED
  • Check to ensure all medication was completed and update Case Manager & copy Supervisor.
  • All CYSW profiles must be 100% complete prior to their first shift.
**Billing
- A key component to ensure a strong relationship with CAS. Billing occurs monthly and must be efficient, organized and most importantly accurate. Updating and verifying shifts daily throughout the month will ensure success at months end.

  • Create SRA Report and ensure form is filled in correctly (all info, details & signature complete)
  • In office billing days occur the 1st of the month or the first Monday of the month if 1st falls on a weekend.
  • Upcoming billing dates are set for, Thursday Sept 1st, Monday Oct 3rd, Tuesday Nov 1st, Thursday Dec 1st, 2022, and into 202
  • You will work along side our bookkeeper to accurately bill CAS the set number of hours and rate of pay from the previous month.

Supervision*:

- **Supervision usually involves solving problem and case specific situations, however; it should also focus on how those problems and work make you feel and how you're impacted by the work

  • Oversee all CYSW and ensure a clinical understanding of the job they do.
  • Regular formal and informal supervision with all active CYSW.
  • Individual CYSW goals may be workrelated or developmental.
  • CYSW of the month: present at Team certificate of the month. Sign letter and mail letter to recipient by months end.
  • Connect with CYSW who did not fill a shift in the month to ensure availability and readdress upcoming openings. Update staff list as needed.

Hiring & Training
- A foundational element within our agency, hiring and developing CYSW are crucial for longevity and consistency with foster homes.

  • Develop and maintain a consistent pool of CYSW's to supports children and teens in our care.
  • Post online, through word of mouth and within our Carpe Diem network to hire competent and strengthbased individuals.
  • CYSW Job postings: update postings for relevance. Book upcoming interviews for office days immediately.

ONCALL:


  • Provide On-Call support for the agency 2 weekends a month (Friday Sunday) or 8 weeks yearly. This will be determined based on the needs of the agency
  • During the weekend of On Call, you will be available by phone to support families and kids in our care.
  • During your full week of oncall, $100 gift card will be provided for each week.

Secondary Responsibilities:


ADMINISTRATION:


  • Attend monthly Team Meetings, monthly clinical meetings and monthly supervision
  • Assist with special events or projects as required
  • Attend training with focus on personal growth for your position
  • Pick up and drop off CD equipment Carpe Diem van can be used if available
  • Outlook calendar is required for scheduling yourself.
