Job summary
The Direct Care Nurse provides care and support for individuals with concurrent psychiatric and/or substance use disorder (SUD), housing and physical health needs directly or in conjunction with an interdisciplinary team or direct supervisor.Responsibilities
- Provides direct health care services to clients presenting with a wide variety and complexity of acute and chronic illnesses/diseases including communicable diseases, addictions, mental health, and general health issues.
- Observes, monitors evaluates assesses documents client progress symptoms behavioral changes performs various interventions utilizing nursing skills medical equipment therapeutic modalities initiates facilitates intake discharge planning identifies referrals other professionals.
